      目的分析血清上皮细胞膜结合黏液素 1(MUC1)、 α1-酸性糖蛋白( AAG)水平与急性胆囊炎病人炎症以及预后的关系。方法收集 2020年 7月至 2022年 4月内蒙古北方医院收治的急性胆囊炎病人 136例,病人根据严重程度分为轻度组、中度组与重度组,另选取同期体检健康者 130例作为对照组,制备研究对象血清标本, ELISA法检测血清 MUC1、AAG水平,比较对照组与急性胆囊炎组、不同严重程度急性胆囊炎病人白细胞计数、中性粒细胞、血清总胆红素、血清 MUC1、AAG、C反应蛋白(CRP)水平;根据急性胆囊炎病人治疗后预后情况将病人分为预后良好组与预后不良组,比较两组血清 MUC1、AAG水平; lo-gistic回归分析血清 MUC1、AAG水平与急性胆囊炎病人预后情况的关系;受试者操作特征( ROC)曲线分析血清 MUC1、AAG水平对急性胆囊炎病人预后不良的预测价值。结果与对照组相比,急性胆囊炎组病人血清 MUC1[( 134.29±22.35)ng/L比

      Objective To analyze the relationship between serum epithelial cell membrane-bound mucin 1 (MUC1) and α1-acid gly-coprotein (AAG) levels and inflammation and prognosis in patients with acute cholecystitis.Methods One hundred and thirty-six pa-tients with acute cholecystitis admitted to the North Hospital of Inner Mongolia from July 2020 to April 2022 were collected. The pa-tients were assigned into mild group, moderate group and severe group according to the severity, another 130 healthy subjects who un-derwent physical examination were selected as the control group. Serum samples of research subjects were prepared, the levels of serumMUC1 and AAG were detected by ELISA, and the levels of white blood cell count, neutrophils, serum MUC1, AAG and C-reactive pro-tein (CRP) levels in patients with acute cholecystitis of different severitywere compared between the control group and the acute chole-cystitis group; according to the prognosis of patients with acute cholecystitis after treatment, the patients were assigned into a good prog-nosis group and a poor prognosis group, and the levels of serum MUC1 and AAG were compared between the two groups. Logistic re-gression was performed to analyze the relationship between serum MUC1 and AAG levels and the prognosis of patients with acute chole-cystitis; receiver operating characteristic (ROC) curve was performed to analyze the predictive value of serum MUC1 and AAG levelsfor poor prognosis in patients with acute cholecystitis.Results Compared with the control group, the serum levels of MUC1 [(134.29± 22.35) ng/L vs. (85.17±14.11) ng/L] and AAG [(0.84±0.14) g/L vs. (0.45±0.07) g/L] in the acute cholecystitis group were significantly in-creased (P<0.05); compared with the mild group, the levels of white blood cell count, neutrophils, serum MUC1, AAG and CRP in themoderate group and severe group were increased (P<0.05); compared with the moderate group, the levels of white blood cell count, neu-trophils,serum MUC1, AAG and CRP in the severe group were increased (P<0.05); compared with the good prognosis group, the serumMUC1 and AAG of the patients with acute cholecystitis in the poor prognosis group were increased (P<0.05); serum MUC1 and AAG were risk factors for poor prognosis in patients with acute cholecystitis (P<0.05); the AUC of serum MUC1, AAG, and their combineddetection for predicting poor prognosis in patients with acute cholecystitis was 0.80 (sensitivity was 61.4 % specificity was 86.4 % ),0.81 (sensitivity was 95.5 %, specificity was 53.4 %), and 0.88 (sensitivity was 88.6 %, specificity was 79.5 %), respectively.Conclu. sion Serum MUC1 and AAG in patients with acute cholecystitis are elevated, and are related to the degree of inflammation, and have a high predictive value for poor prognosis.
