      目的检测可溶性肿瘤坏死因子样凋亡弱诱导因子( sTWEAK)在斑秃病人血清及皮损中的表达水平,并探讨其在斑秃发病过程中的作用及临床意义。方法以 2017年 10月至 2018年 11月在首都医科大学附属北京友谊医院确诊并治疗的斑秃病人 70例为斑秃组,依据病情分为轻症组( 44例)、重症组( 26例)。同期该院因色素痣或皮肤良性肿瘤进行头皮手术的人群 65例作为对照组。比较各组 sTWEAK及相关细胞因子白细胞介素( IL)-4、IL-12、γ干扰素( IFN-γ)蛋白及 mRNA表达水平;采用 Pearson法分析两指标间相关性。多元线性回归分析各指标与 sTWEAK或脱发严重度工具( SALT)评分的关系。结果斑秃组血清及皮损中 sTWEAK表达水平[(196.73±32.48)ng/L,(2.35±0.62)ng/L]高于对照组[(121.75±19.62)ng/L,(1.49±0.45)ng/L](P<0.05)。斑秃组血清及皮损中细胞因子 IL-12、IFN-γ表达水平高于对照组( P<0.05)IL-4表达水平比较差异无统计学意义( P>0.05)。斑秃病人血清及皮损中 sTWEAK表达水平与 IL-12及 IFN-γ呈正相关,与 IL-4,表达水平无明显相关性。轻症组血清及皮损中 sTWEAK[(173.25±26.84)ng/L,(2.04±0.57)ng/L]IL-12及 IFN-γ表达水平低于重症组[(236.47±42.02)ng/L,(2.87±0.70)ng/L](P<0.05);轻症组、重症组血清及皮损中 IL-4表达差异无统计学意义( P>0.05);斑秃病人血清、皮损处 sTWEAK、 IL-12、IFN-γ与 SALT评分均呈正相关(P<0.05)。多元线性回归分析显示, IL-12、IFN-γ与血清和皮损处 sTWEAK呈显著正相关;血清及皮损处 sTWEAK、IL-12、IFN-γ与 SALT评分存在正相关性(P<0.05)。结论斑秃病人血清及皮损中 sTWEAK表达水平增水平、加,与 IL-12及 IFN-γ呈正相关, sTWEAK、IL-12及 IFN-γ均与斑秃病情严重程度密切相关, sTWEAK可能参与斑秃炎症反应过程。

      Objective To detect the expression level of soluble tumor necrosis factor like weak inducer of apoptosis (sTWEAK) in theserum and skin lesions of alopecia areata patients, and to explore its role and clinical significance in the pathogenesis of alopecia areata.Methods A total of 70 patients with alopecia areata who were diagnosed and treated at the Department of Dermatology, BeijingFriendship Hospital, Capital Medical University, between October 2017 and November 2018 were selected for the alopecia areatagroup; these patients were divided into a mild group (44 cases) and a severe group (26 cases) according to their condition. During thesame period, 65 patients who underwent scalp surgery for pigmented nevi or benign skin tumors in the surgery department of the hospital served as the control group. The protein and mRNA expression levels of sTWEAK and the related cytokines interleukin (IL)-4, IL12 and interferon-γ (IFN-γ) were compared among the groups. Pearson's method was used to analyze the correlation between the two indices. Multivariate linear regression was used to analyze the relationship between each index and the sTWEAK score or severity of alopecia tool (SALT) score.Results The levels of sTWEAK expression in serum and skin lesions in the alopecia areata group [(196.73±32.48) ng/L, (2.35±0.62) ng/L] were greater than those in the control group [(121.75±19.62) ng/L, (1.49±0.45) ng/L] (P<0.05). The ex pression levels of the cytokines IL-12 and IFN-γ in the serum and skin lesions of the alopecia areata group were greater than those in the control group (P<0.05), and there was no significant difference in the expression level of IL-4 between the two groups (P>0.05). Theexpression level of sTWEAK in the serum and skin lesions of patients with alopecia areata was positively correlated with that of IL-12 and IFN-γ and was not significantly correlated with the IL-4 expression level (P>0.05). The expression levels of sTWEAK [(173.25± 26.84) ng/L and (2.04±0.57) ng/L], IL-12 and IFN-γ in the serum and skin lesions in the mild group were lower than those in the severegroup [(236.47±42.02) ng/L and (2.87±0.70) ng/L, respectively] (P<0.05). There was no significant difference in the expression levels of IL-4 in the serum or skin lesions between the mild group and severe group (P>0.05). The difference in IL-4 expression was not statistically significant (P>0.05); sTWEAK, IL-12, IFN-γ and SALT scores were positively correlated with the serum and skin lesions of patients with alopecia areata. Multivariate linear regression analysis revealed that IL-12 and IFN-γ were significantly positively correlatedwith serum and sTWEAK in skin lesions; there was a positive correlation between serum and sTWEAK in skin lesions, and between IL12, IFN-γ and the SALT score (P<0.05).Conclusions The expression level of sTWEAK in the serum and skin lesions of patients withalopecia areata increased, and sTWEAK was positively correlated with IL-12 and IFN-γ. sTWEAK, IL-12 and IFN-γ are closely relat ed to the severity of alopecia areata, and sTWEAK may be involved in the inflammatory process of alopecia areata.
