目的探究 2型糖尿病(T2DM)病人血清沉默信息调节因子 2相关酶 1(SIRT1)和叉头样转录因子 O4(FOXO4)表达与糖尿病视网膜病变( DR)的关系。方法采用随机数字表法将 2021年 8月至 2022年 8月在七台河市人民医院收治的 142例 T2DM病人纳入研究,按照国际临床 DR严重程度量表分为无 DR组 62例、非增殖性( NPDR)组 50例和增殖性( PDR)组 30例。采用酶联免疫吸附测定( ELISA)检测血清 SIRT1和 FOXO4的表达水平; Pearson法分析血清中 SIRT1和 FOXO4表达水平的相关性; logistic回归分析影响 T2DM病人 DR发生的因素。 ROC曲线分析血清中 SIRT1和 FOXO4对 T2DM病人 DR发生的诊断价值。结果 PDR组高密度脂蛋白胆固醇( HDL-C)[( 1.22±0.15)mmol/L比( 1.98±0.17)mmol/L、(1.64±0.18)mmol/L]、 SIRT1水平[(3.82±0.50)μg/L比( 5.36±0.56)μg/L、(4.65±0.52)μg/L]显著低于无 DR组及 NPDR组, NPDR组显著低于无 DR组; PDR组收缩压( SBP)、三酰甘油( TG)、 FOXO4[( 14.63±1.48)μg/L比( 12.62±1.45)μg/L、(13.74±1.46)μg/L]水平显著高于无 DR组及 NPDR组, NPDR组显著高于无 DR组( P<0.05)。相关性分析显示,血清 SIRT1和 FOXO4表达水平呈负相关关系( r=-0.47,P<0.001)。 logistic回归分析显示, HDL-C、SIRT1和 FOXO4表达水平是影响 T2DM病人 DR发生的因素( P<0.05)。二者联合诊断 DR发生的 ROC曲线下面积( AUC)高于 SIRT1和 FOXO4单独诊断的 AUC值( Z=32.22,P<0.001;Z=19.03,P<0.001)。结论 T2DM病人血清中 SIRT1表达水平显著降低, FOXO4表达水平显著升高,二者是影响 T2DM病人 DR发生的因素。 |

Objective To investigate the relationship between the expression of silence information regulator 2 homolog 1 (SIRT1)and fork head like transcription factor O4 (FOXO4) in type 2 diabetes mellitus (T2DM) patients and diabetes retinopathy (DR).Meth- ods A total of 142 patients with T2DM admitted to Qitaihe People's Hospital from August 2021 to August 2022 were enrolled by ran-dom number table method, according to the International Clinical DR Severity Scale, 62 cases were included into DR free group, 50cases into non proliferative (NPDR) group and 30 cases into proliferative (PDR) group. The expression levels of SIRT1 and FOXO4 inserum were measured by enzyme linked immunosorbent assay (ELISA); Pearson method was used to analyze the correlation between theexpression levels of SIRT1 and FOXO4 in serum; logistic regression analysis was used to analyze the influencing factors of DR inT2DM patients. ROC curve was used to analyze the diagnostic value of SIRT1 and FOXO4 in serum for DR in T2DM patients.Results The levels of high density lipoprotein cholesterol (HDL-C) [(1.22±0.15) mmol/L vs. (1.98±0.17) mmol/L, (1.64±0.18) mmol/L] and SIRT1 [(3.82±0.50) μg/L vs. (5.36±0.56) μg/L, (4.65±0.52) μg/L] in PDR group were obviously lower than those in non DR group andNPDR group, and those in NPDR group were obviously lower than those in non DR group; the levels of systolic blood pressure (SBP),triglyceride (TG) and FOXO4 [(14.63±1.48) μg/L vs. (12.62±1.45) μg/L, (13.74±1.46) μg/L] in PDR group were obviously higher thanthose in non DR group and NPDR group, and those in NPDR group were obviously higher than those in non DR group (P<0.05). Corre- lation analysis showed that the expression levels of SIRT1 and FOXO4 in serum were negatively correlated (r=-0.47,P<0.001). Logistic regression analysis showed that the expression levels of HDL-C, SIRT1 and FOXO4 were the factors influencing DR in T2DM patients (P<0.05). The area under the ROC curve (AUC) of DR diagnosed jointly by the two methods was higher than that diagnosed separately by SIRT1 and FOXO4 (Z=32.22, P<0.001; Z=19.03, P<0.001).Conclusion The expression level of SIRT1 in serum of T2DM patientsis obviously decreased and the expression level of FOXO4 is obviously increased, they are the factors influencing DR in T2DM patients. |
