      目的探讨长链非编码 RNA小核仁 RNA宿主基因 18(lncRNA SNHG18)在乙型肝炎病毒相关性肝癌( HBV相关 HCC)病人血清中表达变化及与预后的相关性。方法选取 2015年 1月至 2016年 10月西安市第九医院乙型肝炎病人 192例,分为乙型肝炎肝硬化 106例(肝硬化组)乙型肝炎肝癌 86例(肝癌组),同时选取正常对照组 110例。采用实时荧光定量逆转录聚合酶链反应(qRT-PCR)法检测血清中,lncRNA SNHG18相对表达水平;采用 Pearson相关性分析法分析 HBV相关肝癌病人血清 lncRNA SNHG18与甲胎蛋白( AFP)、谷丙转氨酶( ALT)的相关性;采用 Kaplan-Meier法分析 HBV相关肝癌血清 lncRNA SN- HG18表达与病人预后的关系;多因素 Cox回归分析法分析 HBV相关肝癌病人预后的影响因素。结果对照组、肝硬化组、肝癌组血清 lncRNA SNHG18水平( 1.00±0.21、0.75±0.18、0.46±0.09)依次显著降低( P<0.05); TNM分期为 Ⅲ+Ⅳ期、发生淋巴结转移、浸润深层的 HBV相关肝癌病人 lncRNA SNHG18低表达的比例高于 TNM分期为 Ⅰ+Ⅱ期、未发生淋巴结转移、浸润浅层者(P<0.05)。 Pearson相关分析显示, HBV相关肝癌病人血清 lncRNA SNHG18水平与 AFP、ALT均呈负相关( r=-0.41、-0.54,P<0.001); lncRNA SNHG18高表达 HBV相关肝癌病人 5年生存率高于 lncRNA SNHG18低表达病人( P<0.001); lncRNA SNHG18[HR=0.86,95%CI:(0.78,0.96)]、 TNM分期[ HR=2.76,95%CI:(1.13,6.74)]是 HBV相关肝癌病人预后不良的独立影响因素( P<0.05)。结论 HBV相关肝癌病人血清 lncRNA SNHG18低表达,且 lncRNA SNHG18低表达是预后不良的危险因素。

      Objective To investigate the expression changes of long non-coding RNA small nucleolar RNA host gene 18 (lncRNA SNHG18) in serum of patients with hepatitis B virus-related liver cancer (HBV-related HCC) and its correlation with prognosis.Meth- ods One hundred and ninety-two patients with hepatitis B in the Ninth Hospital of Xi'an from January 2015 to October 2016 were se-lected, and they were assigned into 106 cases of hepatitis B cirrhosis, (liver cirrhosis group) and 86 cases of hepatitis B liver cancer(HCC group), meantime, 110 normal control groups were selected. The relative expression level of lncRNA SNHG18 in serum was de-tected by real-time fluorescence quantitative reverse transcription polymerase chain reaction (qRT-PCR) method. The correlation be- tween serum lncRNA SNHG18 and alpha-fetoprotein (AFP) and alanine aminotransferase (ALT) in HBV-associated HCC patients was analyzed by Pearson correlation analysis. Kaplan-Meier method was used to analyze the relationship between the expression of serum lncRNA SNHG18 in HBV-related HCC and the prognosis of patients. Multivariate Cox regression analysis was used to analyze the prog- nostic factors of HBV-related HCC patients.Results The level of serum lncRNA SNHG18 in the control group, liver cirrhosis groupand HCC group was greatly decreased in turn (1.00±0.21, 0.75±0.18, 0.46±0.09, P<0.05); the proportion of low expression of lncRNA SNHG18 of HBV-related HCC patients with TNM stage Ⅲ+Ⅳ, lymph node metastasis, and deep invasion was greatly higher than thatof patients with TNM stage Ⅰ+Ⅱ, no lymph node metastasis, and superficial invasion (P<0.05). Pearson correlation analysis showed that serum lncRNA SNHG18 level in HBV-related HCC patients was negatively correlated with AFP and ALT (r= - 0.41, - 0.54, P< 0.001); the 5-year survival rate of patients with HBV-related HCC with high expression of lncRNA SNHG18 was higher than that of pa- tients with low expression of lncRNA SNHG18 (P<0.001); lncRNA SNHG18 [HR=0.86, 95%CI: (0.78,0.96)] and TNM stage [HR=2.76, 95%CI: (1.13,6.74)] were independent influencing factors of poor prognosis in HBV-related HCC patients (P<0.05).Conclusion The serum lncRNA SNHG18 expression is low in HBV-related HCC patients, and the low expression of lncRNA SNHG18 is a risk factor for poor prognosis.
