      目的探究癫痫病人外周血中三磷酸腺苷结合盒转运子 A7(ABCA7)、髓样细胞触发受体 2(TREM2)的表达水平及与认知功能的关系。方法选取商丘市中心医院 2017年 6月至 2021年 7月收治的 132例癫痫病人为研究对象,根据蒙特利尔认知评估量表( MoCA)得分将癫痫病人分为两组,认知功能正常组( MoCA≥26分, 60例)和认知功能障碍组( MoCA 0~25分, 72例)。采用酶联免疫吸附测定( ELISA)检测血清 ABCA7、TREM2水平; Spearman法分析相关性、 logistic回归分析影响因素、受试者操作特征曲线( ROC曲线)评估预测价值。结果相较于认知功能正常组,认知功能障碍组血清 ABCA7[(1.17±0.26)μg/L比( 1.53±0.37)μg/L]水平升高, TREM2[(25.14±3.47)ng/L比( 20.58±2.52)ng/L]水平降低( t=6.34、8.73,P<0.05)。认知功能障碍病人血清 ABCA7水平与 MoCA评分呈负相关( r=-0.55,P<0.001)血清 TREM2水平与 MoCA评分呈正相关( r=0.56,P<0.001)。 TREM2、发作持续时间、病程、 ABCA7是癫痫病人发生认知功能障碍,的影响因素。血清 ABCA7、TREM2水平单独及联合预测癫痫病人认知功能障碍的曲线下面积分别为 0.79、0.84、0.93,血清 ABCA7、TREM2水平联合预测的曲线下面积明显高于二者单独预测( P<0.05)。结论癫痫病人血清中 ABCA7的表达水平显著升高, TREM2表达水平显著降低,均为癫痫病人发生认知

      Objective To investigate the expression levels of adenosine triphosphate-binding cassette transporter A7 (ABCA7) andtriggering receptor expressed on myeloid cells 2 (TREM2) in the peripheral blood of epilepsy patients and their relationship with cogni-tive function.Methods From June 2017 to July 2021, 132 epilepsy patients admitted to Shangqiu Central Hospital were included asresearch subjects. According to the Montreal Cognitive Assessment Scale (MoCA) scores, the epilepsy patients were divided into twogroups: the normal cognitive function group (MoCA ≥ 26 points, 60 patients) and the cognitive dysfunction group (MoCA 0~25 points,72 patients). Enzyme-linked immunosorbent assay (ELISA) was used to detect serum ABCA7 and TREM2 levels. Spearman's methodwas used to analyze the correlations, logistic regression was used to analyze the influencing factors, and receiver operating characteris-tic (ROC) curves were used to assess the predictive value.Results Compared to those in the group with normal cognitive function, the serum ABCA7 [(1.17±0.26) μg/L vs. (1.53±0.37) μg/L] level in the cognitive impairment group was greater, while the TREM2 [(25.14± 3.47) ng/L vs. (20.58±2.52) ng/L] level was lower (t=6.34, 8.73, P<0.05). The serum ABCA7 level in patients with cognitive dysfunction was negatively correlated with the MoCA score (r=-0.55, P<0.001), and the serum TREM2 level was positively correlated with the Mo- CA score (r=0.56, P<0.001). TREM2, seizure duration, disease duration and ABCA7 were found to influence the development of cogni- tive dysfunction (P<0.05). The areas under the curve for cognitive dysfunction in epileptic patients predicted by serum ABCA7 andTREM2 alone and by both serum ABCA7 and TREM2 were 0.79, 0.84 and 0.93, respectively, and the areas under the curve for cogni-tive dysfunction predicted by the combination of serum ABCA7 and TREM2 were significantly greater than those predicted by either ofthem alone (P<0.05).Conclusion The expression level of ABCA7 in the serum of epileptic patients was significantly greater, whilethat of TREM2 was significantly lower; both of these factors influence the occurrence of cognitive dysfunction and are closely related tothe cognitive function of epilepsy patients.
