      目的探究血清微 RNA(miR)-130b-3p、果蝇母系 DPP同源物 4(SMAD4)对多囊卵巢综合征( PCOS)不孕病人人工授精妊娠结局的预测价值。方法选取 2018年 1月至 2022年 1月在平顶山市妇幼保健院就诊的 PCOS不孕病人 172例为 PCOS组,另取同期体检的 180例月经周期正常且已婚已育女性作为对照组。实时荧光定量 PCR法检测血清中 miR-130b-3p的水平;用酶联免疫吸附测定( ELISA)法检测血清 SMAD4水平。判断 PCOS病人人工授精结局并分为临床妊娠组( n=146)和未临床妊娠组( n=26)。 Pearson法对 PCOS病人血清 SMAD4、miR-130b-3p、抗苗勒氏管激素( AMH)的相关性进行分析; logistic回归分析影响 PCOS不孕病人人工授精后未临床妊娠的危险因素,采用受试者操作特征曲线(ROC曲线)评估 miR-130b-3p、SMAD4水平预测 PCOS病人人工授精未临床妊娠的价值。结果与对照组相比, PCOS组血清 miR-130b-3p(1.02±0.24比 2.16±0.34)水平较高(t=36.47,P<0.05)血清 SMAD4[(71.15±11.62)ng/L比( 36.05±6.37)ng/L]水平较低( t=34.92,P<0.05)与临床妊娠组相比,未临床妊娠组血清 miR-1,30b-3p(2.04±0.31比 2.85±0.51)水平较高( P<0.05),优势卵泡数 ≥2个比例( 78.08%,比 50.00%)及血清 AMH[( 8.27±1.85)pg/L比( 6.16±1.01)pg/L]、 SMAD4[( 38.27±6.99)ng/L比( 23.60±2.91)ng/L]水平较低( P<0.05)。 Pearson分析显示, PCOS病人血清中 SMAD4与 miR-130b-3p呈负相关( r=-0.56,P<0.05),PCOS病人血清中 miR-130b-3p与 AMH呈负相关( r= -0.46,P<0.05)血清中 SMAD4与 AMH呈正相关( r=0.46,P<0.05)。 logistic分析显示, miR-130b-3p水平升高是 PCOS不孕病人人工授精后未临床,妊娠的危险因素( P<0.05)AMH和 SMAD4水平降低是 PCOS不孕病人人工授精后未临床妊娠的保护因素(P<0.05)。 ROC曲线分析显示,血清 miR-130b-,3p、SMAD4联合预测 PCOS病人人工授精未临床妊娠的曲线下面积( AUC)为0.95,其灵敏度为 96.20%,特异度 76.70%。结论 PCOS病人血清 miR-130b-3p水平升高, SMAD4水平下降,二者对 PCOS病人人工授精未临床妊娠有预测价值。

      Objective To investigate the predictive value of serum microRNA (miR)-130b-3p and drosophila mothers against decap-entaplegic 4 (SMAD4) in Drosophila mothers on pregnancy outcomes after artificial insemination in infertile patients with polycysticovary syndrome (PCOS).Methods From January 2018 to January 2022, 172 infertile patients with PCOS treated at the PingdingshanMaternal and Child Health Hospital composed the PCOS group, and another 180 women of child-bearing age with normal menstrual cy-cles who underwent physical examination composed the control group during the same period. The levels of miR-130b-3p in the serum were detected by real-time fluorescence quantitative PCR, and the levels of serum SMAD4 were determined by enzyme-linked immuno- sorbent assay (ELISA). The artificial insemination outcomes of PCOS patients were evaluated, and the patients were grouped into a clin-ical pregnancy group (n=146) and a nonclinical pregnancy group (n=26). The correlation between serum SMAD4, miR-130b-3p and an- ti-Müllerian hormone (AMH) in PCOS patients was analyzed by the Pearson method. Logistic regression was performed to analyze therisk factors affecting nonclinical pregnancies after intrauterine insemination in infertile PCOS patients, and receiver operating charac-teristic (ROC) curves were used to evaluate the value of miR-130b-3p and SMAD4 levels in predicting nonclinical pregnancy after arti- ficial insemination in PCOS patients.Results Compared with those in the control group, the serum levels of miR-130b-3p (1.02±0.24 vs. 2.16±0.34) in the PCOS group were greater (t=36.47, P<0.05), and the serum levels of SMAD4 [(71.15±11.62) ng/L vs. (36.05±6.37) ng/L] were lower (t=34.92, P<0.05). Compared with those in the clinical pregnancy group, the serum levels of miR-130b-3p (2.04±0.31 vs. 2.85±0.51) were greater, while the proportions of dominant follicles ≥ 2(78.08% vs. 50.00%), serum AMH [(8.27±1.85) pg/L vs.(6.16±1.01) pg/L]and SMAD4 [(38.27±6.99) ng/L vs. (23.60±2.91) ng/L] were lower (P<0.05) in the nonclinical pregnancy group. Pear- son analysis revealed that serum SMAD4 was negatively correlated with miR-130b-3p in PCOS patients (r=-0.56, P<0.05), that serum miR-130b-3p was negatively correlated with AMH in PCOS patients (r=-0.46, P<0.05), and that serum SMAD4 was positively correlat- ed with AMH (r=0.46, P<0.05). Logistic regression analysis revealed that the increased level of miR-130b-3p was a risk factor for non- clinical pregnancy after artificial insemination in infertile PCOS patients (P<0.05), and the decreased levels of AMH and SMAD4 wereprotective factors for nonclinical pregnancy after artificial insemination in infertile PCOS patients (P<0.05). ROC analysis revealed that the area under the curve (AUC) of serum miR-130b-3p and SMAD4 combined for the prediction of nonclinical pregnancy after artificialinsemination in patients with PCOS was 0.95, with a sensitivity of 96.20% and a specificity of 76.70%.Conclusion The serum level of miR-130b-3p in PCOS patients was increased, and the level of SMAD4 was decreased; both these factors have predictive value for non-clinical pregnancy in artificial insemination in PCOS patients.
