      目的探究血清长链非编码 RNA肺癌相关转录本 1(lncRNA LUCAT1)、微 RNA-514b-3p(miR-514b-3p)、甲胎蛋白(AFP)表达水平对预测早期肝细胞癌( HCC)病人根治术后复发的临床价值。方法选取 2016年 3月至 2019年 6月西安市中心医院诊治的 130例Ⅰ~Ⅱ期 HCC病人为研究对象,对所有早期 HCC病人随访 3年,按其行根治术后是否复发分为未复发组( n=92)和复发组( n=38)。检测并比较复发组、未复发组早期 HCC病人血清 lncRNA LUCAT1、miR-514b-3p、AFP水平;受试者操作特征曲线( ROC曲线)分析血清 lncRNA LUCAT1、miR-514b-3p、AFP对早期 HCC病人根治术后复发的预测价值;多因素 logistic回归分析早期 HCC病人根治术后复发的影响因素; Pearson法分析根治术后复发的早期 HCC病人血清 lncRNA LUCAT1表达水平与 miR-514b-3p的关系。结果与未复发组[0.99±0.33、(17.42±3.67)μg/L、1.01±0.34]相比,复发组早期 HCC病人血清 lncRNA LUCAT1(1.87±0.63)、 AFP水平( 25.38±5.18)μg/L升高( P<0.05),miR-514b-3p水平( 0.56±0.19)降低( P<0.05)。血清 lncRNA LUCAT1、miR-514b-3p、AFP预测早期 HCC病人根治术后复发的曲线下面积( AUC)分别为 0.81、0.68、0.83,且血清 ln- cRNA LUCAT1、AFP联合预测早期 HCC病人根治术后复发的 AUC为 0.93,灵敏度为 93.8%,特异度为 84.8%。血清 lncRNA LU- CAT1、miR-514b-3p联合预测 AFP阴性病人根治术后复发的 AUC为 0.91,高于二者单独预测( P<0.05);血清 lncRNA LUCAT1、 miR-514b-3p单独及联合预测 AFP阳性病人根治术后复发的 AUC与 AFP单独预测相比,差异无统计学意义(P>0.05)。 AFP、ln- cRNA LUCAT1是影响早期 HCC病人根治术后复发的独立危险因素( P<0.05)miR-514b-3p是独立保护因素( P<0.05)。根治术后复发的早期 HCC病人血清 lncRNA LUCAT1水平与 miR-514b-3p水平呈负相关,(P<0.05),且 lncRNA LUCAT1与 miR-514b-3p存在结合位点。结论根治术后复发的早期 HCC病人血清 lncRNA LUCAT1水平较高, miR-514b-3p水平较低,血清 lncRNA LUCAT1与 AFP联合检测更有利于临床预测早期 HCC病人根治术后复发情况,且血清 lncRNA LUCAT1、miR-514b-3p联合预测 AFP阴性病人根治术后复发的效能更高。

      Objective To investigate the clinical value of serum long noncoding RNA lung cancer-related transcript 1 (lncRNA LU- CAT1), microRNA-514b-3p (miR-514b-3p) and alpha-fetoprotein (AFP) expression levels in predicting the recurrence of early hepato-cellular carcinoma (HCC) patients after radical surgery.Methods A total of 130 patients with stage Ⅰ-Ⅱ HCC who were diagnosedand treated at Xi'an Central Hospital between March 2016 and June 2019 were selected as the study subjects, and all the early HCCpatients who were followed up for 3 years were divided into the nonrecurrent group (n=92) and the recurrent group (n=38) according to whether or not they had relapsed after undergoing radical surgery. The serum levels of the lncRNA LUCAT1, miR-514b-3p and AFPin the recurrent and nonrecurrent groups of early HCC patients were detected and compared; receiver operating characteristic (ROC)curves were used to analyze the predictive value of the serum lncRNA LUCAT1, miR-514b-3p and AFP for the recurrence of early HCC patients after radical surgery. Multivariate logistic regression analysis was made of the factors influencing recurrence after radicalsurgery in early-stage HCC patients. Pearson's method was applied to analyze the relationship between the serum lncRNA LUCAT1expression level and miR-514b-3p in early HCC patients who experienced recurrence after radical surgery.Results Compared withthose in the nonrecurrent group [0.99±0.33, (17.42±3.67) μg/L, 1.01±0.34], early HCC patients in the recurrent group had increasedserum lncRNA LUCAT1 1.87±0.63 and AFP levels (25.38±5.18) μg/L and decreased miR-514b-3p level 0.56±0.19 (P<0.05). The ar- eas under the curves (AUCs) of the serum lncRNA LUCAT1, miR-514b-3p and AFP for predicting postradical recurrence in earlyHCC patients were 0.81, 0.68 and 0.83, respectively, and the AUCs of the combination of the serum lncRNA LUCAT1 and AFP forpredicting postradical recurrence in early-stage HCC patients was 0.93, with a sensitivity of 93.8% and a specificity of 84.8%. TheAUC of the combination of the serum lncRNA LUCAT1 and miR-514b-3p for predicting recurrence after radical surgery in AFP-nega- tive patients was 0.91, which was greater than that predicted by either alone (P<0.05). The AUC of serum lncRNA LUCAT1 and miR514b-3p alone and in combination predicting recurrence after radical surgery in AFP-positive patients were greater than those of AFP alone (P>0.05). The levels of AFP and the lncRNA LUCAT1 were found to be independent risk factors for postradical recurrence inearly HCC patients (P<0.05), and miR-514b-3p was found to be an independent protective factor (P<0.05). Serum lncRNA LUCAT1 level was negatively correlated with the miR-514b-3p level in early HCC patients who experienced recurrence after radical surgery (P< 0.05), and there was a binding site between lncRNA LUCAT1 and miR-514b-3p. Conclusions Serum lncRNA LUCAT1 levels were greater and miR-514b-3p levels were lower in early-stage HCC patients who experienced recurrence after radical surgery. The combi-nation of the serum lncRNA LUCAT1 and AFP is more favorable for the clinical prediction of postradical recurrence in early-stage HCC patients, and the efficacy of the combination of the serum lncRNA LUCAT1 and miR-514b-3p in predicting postradical recur- rence in AFP-negative patients is greater.
