目的研究间歇充气加压联合红花注射液预防腹腔镜胃癌根治术后深静脉血栓的作用及对血清可溶性髓系细胞触发受体 1(sTREM-1)和骨膜蛋白( periostin)的影响。方法选取 2019年 8月至 2022年 10月于武威市凉州医院接受腹腔镜胃癌根治术的 120例病人,采用随机数字表法分为观察组和对照组,每组各 60例。对照组采用间歇充气加压泵方法,每次使用 30 min,每天 2次,持续 10 d。观察组在对照组基础上联合使用红花注射液,静脉滴注,每日 1次,持续 10 d。比较两组深静脉血栓发生情况、凝血功能、炎症因子、 sTREM-1和骨膜蛋白水平。结果治疗期间,对照组有 7例出现深静脉血栓,发生率 11.67%;观察组有 1例出现深静脉血栓,发生率 1.67%。两组比较,差异无统计学意义( P>0.05)。治疗后,两组活化部分凝血活酶时间(APTT)、凝血酶时间( TT)、凝血酶原时间( PT)均降低,且观察组高于对照组( P<0.001);两组纤维蛋白原( FIB)水平均升高,且观察组低于对照组( P<0.001)。治疗后,两组白细胞介素( IL)-6、IL-10和肿瘤坏死因子 -α(TNF-α)水平均升高,且观察组低于对照组( P<0.001)。治疗后,两组 sTREM-1和骨膜蛋白水平均升高,且观察组低于对照组( P<0.001)。结论间歇充气加压联合红花注射液能降低腹腔镜胃癌根治术后深静脉血栓的发生率,改善凝血功能,减轻炎症反应,降低 sTREM-1和骨膜蛋白水平,具有很好的临床应用价值。 |

Objective To study the preventive effect of intermittent pneumatic compression combined with Honghua injection on deep vein thrombosis after laparoscopic radical gastritis for gastric cancer and its effect on serum(sTREM-1) and periostin.Methods A total of 120 patients who underwent laparoscopic radical gastrectomy in Wuwei Liangzhou Hospital from August 2019 to October2022 were selected and randomly assignded into observation group and control group by random number table method, with 60 cases ineach group. The control group was treated with intermittent pneumatic compression pump for 30 min, twice a day for 10 days. The observation group was combined with the control group on the basis of Honghua injection intravenous infusion, once a day, for 10 days.The occurrence of deep vein thrombosis, coagulation function, inflammatory factors, sTREM-1 and periosteum protein levels were compared between the two groups.Results During the treatment period, 7 patients in the control group developed deep vein thrombosis(11.67%). In the observation group, 1 case developed deep vein thrombosis (1.67%). There was no significant difference between thetwo groups (P > 0.05). After treatment, activated partial thrombin time (APTT), thrombin time (TT), and prothrombin time (PT) were reduced in both groups. The observation group was higher than the control group (P < 0.001). The level of fibrinogen (FIB) was increased in both groups, and the observation group was lower than the control group (P < 0.001). After treatment, the levels of interleukin-6, IL10 and tumor necrosis factor-α (TNF-α) were increased in both groups, and the observation group was lower than the control group (P < 0.001). After treatment, sTREM-1 and periosteum protein levels increased in both groups, and the observation group was lower than the control group (P < 0.001). Conclusion Intermittent pneumatic compression combined with Honghua injection can reduce the incidence of deep vein thrombosis after laparoscopic radical gastrectomy, improve coagulation function, reduce inflammation, and reducesTREM-1 and periosteum protein levels, which has a good clinical application value. |
