      目的探讨血清白细胞介素 -34(IL-34)、脂肪细胞型脂肪酸结合蛋白 4(FABP4)对支气管哮喘病儿病情及控制水平的评估价值。方法收集聊城市第二人民医院 2020年 6月至 2023年 1月治疗的 97例支气管哮喘病儿为病例组,依据病儿病情程度分为轻度哮喘组 32例、中度哮喘组 41例、重度哮喘组 24例,依据病儿病情控制水平分为控制组 56例和未控制组 41例;另选取 95例体检健康儿童作为对照组,比较各组血清 IL-34、FABP4水平;采用 logistic回归分析支气管哮喘病儿病情控制水平的影响因素;采用受试者操作特征曲线(ROC曲线)分析血清 IL-34、FABP4水平对支气管哮喘病儿病情未控制的评估价值。结果病例组血清 IL-34[(25.74±7.31)ng/L比( 2.46±0.73)ng/L]、 FABP4[(27.82±4.10)μg/L比(11.69±3.07)ng/L]水平高于对照组( P<0.05)。重度哮喘组血清 IL-34[( 34.78±9.31)ng/L比( 25.82±7.26)ng/L比( 18.79±6.64)ng/L]、 FABP4[( 37.14±7.40)μg/L比( 28.04±4.75)μg/L比(20.55±5.15)μg/L]水平高于中度哮喘组和轻度哮喘组,中度哮喘组血清 IL-34、FABP4水平高于轻度哮喘组(P<0.05)。未控制组血清 IL-34、FABP4水平及治疗依从性差、有呼吸道感染的病儿比例高于控制组(P<0.05)。IL-34、FABP4是支气管哮喘病儿病情未控制的独立危险因素(P<0.05)。血清 IL-34、FABP4水平单独及联合评估支气管哮喘病儿病情未控制的 AUC分别为 0.78、0.85、0.93。结论血清 IL-34、FABP4水平与支气管哮喘病儿病情程度密切相关,可较好地评估病情控制水平。

      Objective To explore the evaluation value of serum interleukin-34 (IL-34) and adipocyte fatty acid binding protein 4 (FABP4) for the condition and control level of bronchial asthma in children.Methods A total of 97 children with bronchial asthma treated in Liaocheng Second People's Hospital from June 2020 to January 2023 were collected as a case group, according to the severity of the patient's condition, they were grouped into a mild asthma group of 32 cases, a moderate asthma group of 41 cases, and a severeasthma group of 24 cases, according to the level of disease control, the children were grouped into a control group of 56 cases and an uncontrolled group of 41 cases; another 95 healthy children undergoing physical examination were collected as the control group, the levels of IL-34 and FABP4 in serum of each group were compared; Logistic regression analysis was applied to analyze influence factors onthe level of disease control in children with bronchial asthma; receiver operating characteristic (ROC) curve was applied to analyze theevaluation value of serum IL-34 and FABP4 levels for uncontrolled condition in children with bronchial asthma.Results The serum levels of IL-34 [(25.74±7.31) ng/L vs.(2.46±0.73) ng/L] and FABP4 [(27.82±4.10)μg/L vs. (11.69±3.07) ng/L] in the case group were higher than those in the control group (P<0.05). The serum levels of IL-34 [(34.78±9.31) ng/L vs. (25.82±7.26) ng/L vs. (18.79±6.64) ng/ L] and FABP4 [(37.14±7.40) μg/L vs. (28.04±4.75) μg/L vs. (20.55±5.15) μg/L] in the severe asthma group were higher than those inthe moderate and mild asthma groups, while the serum levels of IL-34 and FABP4 in the moderate asthma group were higher than those in the mild asthma group (P<0.05). The levels of IL-34 and FABP4 in the serum, and the proportions of children with poor treatmentcompliance and respiratory infections in the uncontrolled group were higher than those in the control group (P<0.05). IL-34 and FABP4 were independent risk factors for uncontrolled condition in children with bronchial asthma (P<0.05). The AUC of uncontrolled condition in children with bronchial asthma evaluated by serum IL-34 and FABP4 levels alone and in combination was 0.78, 0.85, and 0.93, respectively.Conclusion The levels of serum IL-34 and FABP4 are closely related to the severity of bronchial asthma in children, andcan effectively evaluate the level of disease control.
