      目的探讨低级别胶质瘤病人异柠檬酸脱氢酶 -1(IDH-1)基因分型与磁共振表观弥散系数( ADC)、血管内皮生长因子(VEGF)表达的关系。方法纳入新疆医科大学附属肿瘤医院 2021年 6月至 2023年 5月收治的低级别脑胶质瘤病人 52例开展回顾性研究,病人术前均接受磁共振弥散加权成像检查,经手术病理与 WHO中枢神经系统肿瘤分级标准证实为低级别脑胶质瘤,根据 IDH-1分型结果分成突变型组 29例、野生型组 23例。比较两组最小 ADC值( ADCmin)、平均 ADC值(ADCmean)、最小相对 ADC值( rADCmin)、平均相对 ADC值( rADCmean)。绘制受试者操作特征曲线( ROC曲线)分析 ADC值对 IDH-1分型的鉴别价值。分析病人 IDH-1分型与 VEGF表达的关系,比较不同 VEGF表达病人的 ADC值。结果突变型组 ADCminADCmeanrADCmin、 rADCmean较野生型组增高( P<0.05)。 ADCminADCmeanrADCminrADCmean鉴别IDH-1分型的曲线下面积(AUC)分别、为 0.77(灵敏度75.86%,特异度 73.91%)、 0.79(灵敏度 82.7678.26.76(灵敏度 79.31%,特异度 78.26%)、 0.77(灵敏度 79.31%,特异度 82.61%)。突变型组 VEGF阳性率为 13.79%,低于野生型组的 47.83%(P<0.05)。 VEGF阴性 /弱阳性组 ADCmin、ADCmean、rADCmin、rADCmean高于阳性组( P<0.05)。结论低级别脑胶质瘤 IDH-1突变者的 ADCmin、ADCmean、rADCmin、rADCmean相对高, ADC参数对其 IDH-1分型具有鉴别价值,且 IDH-1分型与 VEGF表达相关。

      Objective To investigate the relationship between isocitrate dehydrogenase 1 (IDH-1) genotyping and the expression ofmagnetic resonance apparent diffusion coefficient (ADC) and vascular endothelial growth factor (VEGF) in low-grade glioma patients. Methods A retrospective study was carried out on 52 patients with low-grade glioma admitted to Cancer Hospital Affiliated to Xinjiang Medical University from June 2021 to May 2023. All patients were examined by diffusion-weighted magnetic resonance imaging before surgery, and were confirmed as low-grade glioma by surgical pathology and WHO central nervous system tumor grading criteria. According to the results of IDH-1, they were divided into mutant group (n=29) and wild type group (n=23). The minimum ADC value(ADCmin), average ADC value (ADCmean), minimum relative ADC value (rADCmin) and average relative ADC value (rADCmean) ofthe two groups were compared. Receiver operating characteristic (ROC) curve was drawn to analyze the value of ADC value in differentiating IDH-1 typing. The relationship between IDH-1 type and VEGF expression was analyzed, and ADC values of patients with different VEGF expression were compared.Results ADCmin, ADCmean, rADCmin and rADCmean in mutant group were higher than those in wild type group (P<0.05). The area under the curve (AUC) of ADCmin, ADCmean, rADCmin and rADCmean for identification of IDH-1 typing were 0.77 (sensitivity 75.86%, specificity 73.91%), 0.79 (sensitivity 82.76%, specificity 73.91%), 0.76 (sensitivity79.31%, specificity 78.26%) and 0.77 (sensitivity 79.31%, specificity 82.61%), respectively. The positive rate of VEGF in mutantgroup was 13.79%, lower than 47.83% in wild type group (P<0.05). ADCmin, ADCmean, rADCmin and rADCmean in negative/weakpositive VEGF group were higher than those in positive group (P<0.05).Conclusion ADCmin, ADCmean, rADCmin and rADCmean are relatively high in low-grade glioma with IDH-1 mutation, and ADC parameters are valuable in differentiating IDH-1 classification, and IDH-1 classification is correlated with VEGF expression.
