      目的探讨长链非编码 RNA(lncRNA)HOTAIR及 lncRNA RAB27A与慢性阻塞性肺疾病( COPD)严重程度的关系和预测 COPD急性加重期(AECOPD)死亡的价值。方法选取 2020年 1月至 2022年 12月儋州市人民医院收治的 COPD病人 205例和体检正常者 50例(健康对照组)检测所有研究对象血清白细胞介素( IL)-1β、IL-6、肿瘤坏死因子 α(TNF-α)、 lncRNA HO-TAIR及 RAB27A水平,记录肺功能指标,。根据 COPD病人急性加重情况分为稳定期 COPD组( 98例)和 AECOPD组( 107例)根据 GOLD分级标准将 COPD病人分为轻度 48例、中度 74例,重度 63例和极重度 20例。根据 107例 AECOPD病人住院 28 d内发,生死亡情况,将其分为存活组( 63例)和死亡组( 44例)。应用多因素 logistic回归分析影响 AECOPD病人死亡的危险因素,以受试者操作特征曲线( ROC曲线)分析 lncRNA HOTAIR及 RAB27A预测 AECOPD病人死亡的价值。采用 Pearson相关分析检验 AECOPD病人 lncRNA HOTAIR及 RAB27A表达水平与炎症因子及肺功能指标的相关性。结果 AECOPD组血清 lncRNA HO-TAIR(3.18±1.45比 1.96±0.91,1.13±0.24)及 RAB27A(1.90±0.84比 1.07±0.15,0.46±0.08)表达水平明显高于稳定期 COPD组和健康对照组( P<0.05)且稳定期 COPD组高于健康对照组( P<0.05)。极重度组和重度组血清 lncRNA HOTAIR(4.10±1.72,2.97± 1.34比 2.05±0.96,1.82,±0.75)及 RAB27A(2.61±1.05,1.73±0.62比 1.12±0.18,0.86±0.10)表达水平均明显高于轻度组和中度组(P< 0.05)且极重度组高于重度组( P<0.05)。死亡组血清 lncRNA HOTAIR(3.80±1.63比 2.61±1.20)及 RAB27A(2.36±0.98比 1.53± 0.47)表,达水平明显高于存活组( P<0.05)。多因素 logistic回归分析显示,血清 lncRNA HOTAIR[OR=3.38,95%CI:(2.49, 10.11)]及 lncRNA RAB27A[OR=2.90,95%CI:(2.22,9.42)]表达水平升高是影响 AECOPD病人死亡的危险因素( P<0.05)。 ROC曲线分析显示, lncRNA HOTAIR≥3.28及 RAB27A≥2.01联合预测 AECOPD病人死亡的曲线下面积( AUC)高达 0.94[95%CI:(0.88,0.99)],灵敏度为 97.2%,特异度为 84.8%。相关性分析显示, AECOPD病人 lncRNA HOTAIR及 RAB27A表达水平与 IL-1β、IL-6及 TNF-α均呈正相关( P<0.05)而与呼气峰流速占预计值百分比( PEF% pred)、第 1秒用力呼气容积占预计值百分比(FEV1% pred)及一秒率均呈负相关(P<0.0,5)。结论血清 lncRNA HOTAIR及 RAB27A在 COPD病人中呈高表达,与其疾病严重程度及预后不良有关, lncRNA HOTAIR及 RAB27A联合预测 AECOPD死亡具有较好的参考价值。

      Objective To investigate the relationship between long-chain non-coding RNA HOTAIR (lncRNA HOTAIR) and long-chain non-coding RNA RAB27A (lncRNA RAB27A) and the severity of chronic obstructive pulmonary disease (COPD) and their valuein predicting the death of acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods Totally 205 patients withCOPD and 50 patients with normal physical examination (control group) admitted to Danzhou People's Hospital from January 2020 toDecember 2022 were selected. The levels of serum interleukin-1β (IL-1β), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), ln-cRNA HOTAIR and RAB27A were detected in all subjects, and lung function indexes were recorded. According to the acute exacerba-tion of COPD, the patients were assigned into stable COPD group (n=98) and AECOPD group (n=107). According to the GOLD gradingstandards, the patients were assigned into mild 48 cases, moderate 74 cases, severe 63 cases and extremely severe 20 cases. Accordingto the deaths of 107 AECOPD patients within 28 days of hospitalization, they were assigned into a survival group (n=63) and a death group (n=44). Multivariate logistic regression was used to analyze the risk factors affecting the death of patients with AECOPD. Receiv-er operating characteristic curve (ROC) were drawn to analyze the value of lncRNA HOTAIR and RAB27A in predicting the death ofpatients with AECOPD. Pearson correlation was used to analyze the correlation between the expression levels of lncRNA HOTAIR and RAB27A and the inflammatory factors and lung function indicators in AECOPD patients.Results The expression levels of serum ln-cRNA HOTAIR (3.18 ± 1.45 vs. 1.96 ± 0.91, 1.13 ± 0.24) and RAB27A (1.90 ± 0.84 vs. 1.07 ± 0.15, 0.46 ± 0.08) in AECOPD group were significantly higher than those in stable COPD group and control group (P<0.05), and the expression levels in stable COPD group was higher than that in control group (P<0.05). The expression levels of serum lncRNA HOTAIR (4.10±1.72, 2.97±1.34 vs. 2.05±0.96, 1.82±0.75) and RAB27A (2.61±1.05, 1.73±0.62 vs. 1.12±0.18, 0.86±0.10) in the extremely severe group and severe group were signifi-cantly higher than those in the mild and moderate group (P<0.05), and the expression levels in the extremely severe group was higher than those in the severe group (P<0.05). The expression levels of serum lncRNA HOTAIR (3.80±1.63 vs. 2.61±1.20) and RAB27A (2.36±0.98 vs. 1.53±0.47) in the death group were significantly higher than those in the survival group (P<0.05). Multivariate logistic re-gression analysis results showed that the increased expressions of serum lncRNA HOTAIR [OR=3.38,95%CI:(2.49,10.11)] and lncRNA RAB27A [OR=2.90,95%CI:(2.22,9.42)] were risk factors for the death of AECOPD patients (P<0.05). ROC analysis showed that the ar-ea under the curve (AUC) of lncRNA HOTAIR≥ 3.28 and RAB27A≥ 2.01 combined to predict the death of AECOPD patients was ashigh as 0.94 [95%CI:(0.88,0.99)], with sensitivity of 97.2% and specificity of 84.8%. Correlation analysis showed that the expressionlevels of lncRNA HOTAIR and RAB27A in AECOPD patients were positively correlated with IL-1β, IL-6 and TNF-α(P<0.05), but negatively correlated with PEF% pred, FEV1% pred and FEV1/FVC (P<0.05). Conclusions The serum lncRNA HOTAIR and RAB27A are highly expressed in COPD patients, which is related to the severity of the disease and poor prognosis. The combination oflncRNA HOTAIR and RAB27A has good reference value for predicting AECOPD mortality.
