      目的探讨血清自噬相关蛋白 Beclin1水平与糖尿病病人颈动脉粥样硬化和颈动脉内中膜厚度( CIMT)的相关性。方法前瞻性选取 2017年 3月至 2020年 12月于济南市妇幼保健院就诊的 126例糖尿病病人为研究对象,就诊时测量病人血清 Beclin1水平及 CIMT水平,记录病人颈动脉粥样硬化情况,并根据病人血清 Beclin1水平的中位数分为高水平组和低水平组;分析血清 Beclin1水平与糖尿病颈动脉粥样硬化和 CIMT水平的相关性;采用逐步回归评估 CIMT的独立危险因子,构建受试者操作特征曲线以确定灵敏度和特异度。结果血清 Beclin1水平的中位数为 2.16 μg/L,根据病人血清 Beclin1水平的中位数分为高水平组和低水平组,每组 63例。高水平组病人空腹血糖、糖化血红蛋白、三酰甘油、总胆固醇、低密度脂蛋白、 CTMT[( 5.89±0.95)mm比( 7.46±1.10)mm]、颈动脉粥样硬化发生率[17.46%(11/63)比 33.33%(21/63)]均低于低水平组,高密度脂蛋白高于低水平组( P<0.05); Pearson相关分析法分析显示血清 Beclin1水平与糖尿病颈动脉粥样硬化和 CIMT水平呈负相关( r= .0.42,P<0.001;r=.0.54,P<0.001)并与三酰甘油、总胆固醇、低密度脂蛋白也呈负相关(r=.0.42,P<0.001;r=.0.34,P<0.001;r= .0.52,P<0.001),与空腹血糖、糖化血,红蛋白水平呈弱负相关( r=.0.31,P<0.001;r=.0.25,P=0.005);与高密度脂蛋白呈正相关(r=0.38,P<0.001); logistic回归分析显示血清 Beclin1水平升高( OR=0.32)是 CIMT>5 mm的独立保护因素,最佳截断值为 2.35 μg/L,曲线下面积为 0.88,灵敏度和特异度分别为 98.00%、70.00%。结论血清 Beclin1水平与糖尿病病人颈动脉粥样硬化和 CIMT水平呈负相关, CIMT>5 mm的最佳血清 Beclin1截断值为 2.35 μg/L。

      Objective To investigate the correlation between serum autophagy-related protein Beclin1 level and carotid atherosclero-sis and carotid intima-media thickness (CIMT) level in diabetic patients.Methods A total of 126 diabetic patients in Ji'nan Maternaland Child Health Hospital from March, 2017 to December, 2020 were prospectively selected as the research objects. The serum Be-clin1 level and CIMT level of patients were measured at the time of treatment, and the carotid atherosclerosis of patients was recorded,and they were divided into high-level group and low-level group according to the median serum Beclin1 level of patients. The correla-tion between serum Beclin1 level and diabetic carotid atherosclerosis and CIMT level was analyzed; The independent risk factors ofCIMT were evaluated by stepwise regression, and the receiver operation characteristic curve was constructed to determine the sensitivi-ty and specificity.Results The median level of serum Beclin1 was 2.16 μg/L, and patients were divided into high-level group and low-level group according to the median level of serum Beclin1, with 63 cases in each group. In the high level group, the fasting blood glu-cose, glycosylated hemoglobin, triglyceride, total cholesterol, low density lipoprotein, CIMT [(5.89±0.95)mm vs. (7.46±1.10)mm] and the incidence of carotid atherosclerosis [17.46% (11/63) vs. 33.33% (21/63)] was lower than that of low-level group, the high-level HDL was higher than that in the low-level HDL group (P<0.05). Pearson correlation analysis showed that serum Beclin1 level was negative-ly correlated with diabetic carotid atherosclerosis and CIMT level (r=.0.42, P<0.001; r=.0.54, P<0.001), and was negatively correlated with triglyceride, total cholesterol and low density lipoprotein (r= . 0.42, P<0.001; r= . 0.34,P<0.001; r=. 0.52, P<0.001), which was weakly negatively correlated with fasting blood glucose and glycosylated hemoglobin (r=.0.31, P<0.001; r=.0.25,P=0.005); It was posi-tively correlated with high density lipoprotein (r=0.38, P<0.001). Logistic regression analysis showed that the increase of serum Beclin1 level (OR=0.32) was an independent protective factor for CIMT>5 mm, and the optimal cut-off value was 2.35 μg/L, the area under the curve was 0.88, and the sensitivity and specificity were 98.00% and 70.00%, respectively.Conclusions The level of serum Beclin1 is negatively correlated with carotid atherosclerosis and CIMT level in diabetic patients. The optimal cut-off value of serum Beclin1 with CIMT>5 mm is 2.35 μg/L.
