      目的分析 FK506结合蛋白 5(FKBP5)基因多态性与抑郁症易感性及自杀行为的关系。方法纳入 2018年 1月至 2022年 1月商丘市第二人民医院收治的抑郁症病人 170例,设置为抑郁症组。纳入同期于该院体检的健康受试者 150例,设为健康组。对两组 FKBP5基因的单核苷酸多态性(SNP)位点 rs1360780、rs3800373、rs4713916、rs9470080的多态性分布频率进行 Hardy-Weinberg平衡检验。根据抑郁症组是否存在自杀行为将其分为自杀行为组( 33例)、无自杀行为组( 117例)。比较抑郁症组、健康组 rs1360780、rs3800373、rs4713916、rs9470080位点的基因多态性;自杀行为组、无自杀行为组 rs1360780、 rs3800373、rs4713916、rs9470080位点的基因多态性;采用 logistic回归模型分析 FKBP5基因 SNP位点与抑郁症易感性及自杀行为的关系。结果抑郁症组、健康组的 FKBP5基因 rs1360780、rs3800373、rs4713916、rs9470080位点的实际基因与 Hardy-Weinberg遗传平衡定律预测的基因型频率比较,均差异无统计学意义( P>0.05)。与健康组( 13.33%、34.33%)比较,抑郁症组 rs1360780、rs3800373位点上 TT基因型频率、 T等位基因频率( 46.67%、69.33%)均升高( P<0.05)。 logistic回归分析显示,家族史[OR=2.54,95%CI:(1.54,4.21)]、严重睡眠障碍[OR=6.51,95%CI:(2.67,15.92)]、自评经济状况[OR=1.92,95%CI:(1.26, 2.93)]、合并慢性病[OR=3.01,95%CI:(1.06,8.51)]及 FKBP5基因 rs1360780、rs3800373位点上 TT基因型[OR=2.78,95%CI:(1.55,4.98); OR=3.08,95%CI:(1.49,6.39)]是抑郁症易感性的影响因素( P<0.05)。与无自杀行为组( 8.55%、42.74%、21.37%、 22.22%、40.17%、66.24%)比较,自杀行为组家族史、重大负性生活事件、自责、绝望百分率及 rs1360780位点上 TT基因型频率、 T等位基因频率( 27.27%、72.73%、42.42%、51.52%、69.70%、80.30%)升高( P<0.05)。 logistic回归模型分析显示,家族史[OR=2.10,95%CI:(1.17,3.76)]、重大负性生活事件[OR=3.50,95%CI:(1.70,7.22)]、自责[OR=2.79,95%CI:(1.13,6.92)]、绝望[OR=6.60,95%CI:(2.50,17.41)]及 FKBP5基因 rs1360780位点上 TT基因型[OR=1.99,95%CI:(1.32,3.00)]是抑郁症自杀行为的影响因素( P<0.05)。结论 FKBP5基因 rs1360780、rs3800373位点 TT基因型频率、 T等位基因频率可能与抑郁症易感性有关,且 rs1360780位点 TT基因型频率、 T等位基因频率可能增加病人自杀风险。

      Objective To analyze the relationship between FK506 binding protein 5 (FKBP5) gene polymorphism and depression sus-ceptibility and suicidal behavior.Methods One hundred and seventy patients with depression admitted to the Second People's Hospi-tal of Shangqiu City from January 2018 to January 2022 were enrolled as depression group. One hundred and fifty healthy subjects whounderwent physical examination in the hospital during the same period were included as the normal group. The distribution frequencyof single nucleotide polymorphism (SNP) sites rs1360780, rs3800373, rs4713916 and rs9470080 of FKBP5 gene in two groups weretested by Hardy-Weinberg balance test. The depression group was divided into suicidal behavior group (33 cases) and non-suicidal be-havior group (117 cases) according to whether there was suicidal behavior. The polymorphisms of rs1360780, rs3800373, rs4713916and rs9470080 in depression group and normal group, as well as the polymorphisms of rs1360780, rs3800373, rs4713916, rs9470080in suicidal behavior group and non-suicidal behavior group, were compared. Logistic regression model was used to analyze the relation-ship between the SNP locus of FKBP5 gene and depression susceptibility and suicidal behavior.Results There were no significant dif-ferences in the frequency of FKBP5 gene rs1360780, rs3800373, rs4713916 and rs9470080 compared with that predicted by Hardy-Weinberg law of genetic balance between depression group and normal group (P>0.05). Compared with normal group (13.33% and34.33%), TT genotype frequency and T allele frequency (46.67% and 69.33%) at rs1360780 and rs3800373 sites in depression group were increased (P<0.05). Logistic regression analysis showed that family history [OR=2.54, 95%CI: (1.54, 4.21)], severe sleep disorder [OR=6.51, 95%CI: (2.67, 15.92)], self-rated economic status [OR=1.92, 95%CI: (1.26, 2.93)], chronic diseases [OR=3.01, 95%CI: (1.06, 8.51)] and TT genotype at rs1360780 and rs3800373 of FKBP5 gene [OR=2.78, 95%CI: (1.55, 4.98), OR=3.08, 95%CI: (1.49, 6.39)] were the influencing factors of depression susceptibility (P<0.05). Compared with the group without suicidal behavior (8.55%,42.74%, 21.37%, 22.22%, 40.17%, 66.24%), family history, major negative life events, remorse, despair, TT genotype frequency and Tallele frequency (27.27%, 72.73%, 42.42%, 51.52%, 69.70%, 80.30%) at rs1360780 were increased in suicidal behavior group (P< 0.05). Logistic regression model analysis showed that family history [OR=2.10, 95%CI: (1.17, 3.76)], major negative life events [OR= 3.50, 95%CI: (1.70, 7.22)], self-blame [OR=2.79, 95%CI: (1.13, 6.92)], despair [OR=6.60, 95%CI: (2.50, 17.41)] and TT genotype at rs1360780 locus of FKBP5 gene [OR=1.99, 95%CI: (1.32, 3.00)] were the influential factors of depressive suicidal behavior (P<0.05). Conclusion TT genotype frequency and T allele frequency at rs1360780 and rs3800373 of FKBP5 gene may be related to depressionsusceptibility, and TT genotype frequency and T allele frequency at rs1360780 may increase the risk of suicide.
