      目的探究纤溶酶原激活物抑制剂 -1(PAI-1)和 Toll样受体 4(TLR4)在烧伤后增生性瘢痕( HS)组织中的表达水平及临床意义。方法选取河南科技大学第一附属医院 2020年 7月至 2022年 7月收治的 94例 HS病人为研究对象,术中取病人 HS组织为 HS组,并选取 HS病人正常皮肤组织为对照组。实时荧光定量逆转录聚合酶链反应( qRT-PCR)法对组织中 PAI-1、 TLR4 mRNA表达水平进行测定;免疫组化法对组织中 PAI-1、TLR4阳性表达情况进行测定;采用蛋白质印迹法测定 PAI-1、 TLR4的蛋白表达水平;使用温哥华瘢痕量表( VSS)对瘢痕组织严重情况进行评价,使用跨皮水分丢失( TEWL)对 HS病人的表皮屏障功能进行评估; Pearson相关性分析病人 HS组织中 PAI-1、TLR4表达水平与 VSS及 TEWL净丧失量之间的关系。结果 HS组 PAI-1表达水平 1.54±0.32、TLR4 mRNA表达水平 1.61±0.38高于对照组 1.11±0.24、1.06±0.21,HS组 PAI-1、TLR4阳性表达率及蛋白表达水平也显著高于对照组( P<0.05);病人 HS组织 VSS评分为( 12.01±3.48)分, HS组织 TEWL值显著高于对照组( P<0.05)TEWL净丧失量为( 7.87±2.37)g·m.2 ·h.1;Pearson相关性分析结果表明, HS组织中 PAI-1、TLR4表达水平分别与 VSS及 TEWL丧失量呈显著正相关(P<0.05)。结论烧伤后病人 HS组织中 PAI-1、TLR4 mRNA表达水平、阳性表达率及蛋白表达水净,平均显著升高,且 PAI-1、TLR4与 HS组织严重程度,表皮屏障功能障碍有关,因此 PAI-1、TLR4有望成为临床治疗 HS的新靶点。

      Objective To investigate the expression levels and clinical significance of plasminogen activator inhibitor-1 (PAI-1) and Toll-like receptor 4 (TLR4) in hypertrophic scar (HS) tissue after burn. Methods Ninety-four patients with HS who were admitted tothe First Affiliated Hospital of He'nan University of Science and Technology from July 2020 to July 2022 were selected as the study sub-jects. The HS tissues of patients were selected as the HS group during the operation, and the normal skin tissues of HS patients were se-lected as the control group. The expression levels of PAI-1 and TLR4 mRNA in tissues were measured by fluorescence quantitative PCR (qRT-PCR); the positive expression of PAI-1 and TLR4 in tissues was determined by immunohistochemistry; Western blotting was ap-plied to determine the protein expression levels of PAI-1 and TLR4; the severity of scar tissue was evaluated using the Vancouver ScarScale (VSS), and the epidermal barrier function of HS patients was evaluated using transcutaneous water loss (TEWL); Pearson correla-tion was applied to analyze the relationship between the expression levels of PAI-1 and TLR4 in HS tissue and the VSS and net loss of TEWL. Results PAI-1 (1.54±0.32) and TLR4 (1.61±0.38) mRNA expression levels in HS group were higher than those in controlgroup (1.11±0.24) and (1.06±0.21), and the positive expression rates and protein expression level of PAI-1 and TLR4 in HS group were also obviously higher than those in control group (P<0.05); the VSS score of HS tissue was (12.01±3.48), and the TEWL value of HS tis-sue was obviously higher than that of the control group (P<0.05), the net loss of TEWL was (7.87±2.37) g·m.2·h.1; Pearson correlation analysis showed that the expression levels of PAI-1 and TLR4 in HS tissue were obviously positively correlated with the VSS and net loss of TEWL, respectively (P<0.05). Conclusion The mRNA expression levels, positive expression rates and protein expression of PAI-1 and TLR4 in HS tissue of burn patients are obviously increased, PAI-1 and TLR4 are related to the severity of HS and the dysfunction of epidermal barrier, therefore, PAI-1 and TLR4 are expected to become new targets for clinical treatment of HS.
