      目的检测老年冠心病病人血清中分泌型糖蛋白神经生长导向因子 2(Slit2)蛋白表达水平,分析其与病人病情严重程度的相关性。方法回顾性收集 2020年 1月至 2022年 1月三二〇一医院收治的老年冠心病病人 127例为研究组,另取该院同期体检健康者 80例为对照组。采用冠脉狭窄程度( Gensini)分级将研究组分为轻度病变组 52例、中度病变组 43例、重度病变组 32例。收集病人的一般临床资料,检测血清 Slit2蛋白水平;采用 Spearman等级相关分析血清 Slit2蛋白与冠心病病人病情严重程度的相关性; logistic回归分析老年冠心病冠状动脉重度病变的影响因素;受试者操作特征曲线( ROC曲线)分析血清 Slit2蛋白表达水平对冠心病冠状动脉重度病变的预测效能。结果轻度病变组、中度病变组、重度病变组病人总胆固醇(TC)、三酰甘油( TG)、低密度脂蛋白胆固醇( LDL-C)水平显著高于对照组,并且 LDL-C水平随着病变程度的加重而增高( P< 0.05)。研究组血清 Slit2蛋白表达水平[(1.94±0.45)μg/L]显著低于对照组[(3.52±0.80)μg/L](P<0.05)。轻度[(2.47±0.55)μg/ L]、中度[(1.85±0.41)μg/L]、重度病变组[(1.20±0.34)μg/L]病人血清 Slit2蛋白表达水平依次降低( P<0.05)。 Spearman分析显示,冠心病病人血清中 Slit2蛋白表达水平与病情严重程度呈负相关( r=.0.53,P<0.05)。多因素 logistic回归分析显示,血清 Slit2蛋白和 LDL-C是老年冠心病冠状动脉重度病变的影响因素( P<0.05)。血清 Slit2预测老年冠心病冠状动脉重度病变的 ROC曲线下面积( AUC)为 0.93,灵敏度为 93.75%,特异度为 88.37%,截断值为 1.54 μg/L,约登指数为 0.82,LDL-C预测冠状动脉重度病变的 AUC为 0.89,灵敏度为 87.50%,特异度为 86.05%,截断值为 3.48 mmol/L,约登指数为 0.74,二者联合预测冠状动脉重度病变的 AUC为 0.96,灵敏度为 96.87%,特异度为 88.37%,约登指数为 0.85。Slit2单独预测冠状动脉重度病变的 AUC值与 LDL-C单独预测 AUC值差异无统计学意义(P>0.05)但二者联合预测 AUC值大于 LDL-C预测冠状动脉重度病变的 AUC(Z=s2.10,P=0.036),且灵敏度较高。结论老年冠心病病清 Slit2蛋白表达水平下降,与病变严重程度相关,可预测老年冠心病冠状动脉重度病变,临床应用价值较高。

      Objective To detect the expression level of Slit2 protein in the serum of elderly patients with coronary heart disease, andto analyze its correlation with the severity of the disease.Methods A total of 127 elderly patients with coronary heart disease admittedto 3201 Hospital from January 2020 to January 2022 were gathered as the research group, another 80 healthy people who underwentphysical examination in our hospital were gathered as the control group. According to degree of coronary stenosis (Gensini) classifica-tion, the study group was divided into mild lesion group (52 cases), moderate lesion group (43 cases) and severe lesion group (32 cases).General clinical data of patients were collected and serum Slit2 protein levels were detected. Spearman rank correlation was used to an-alyze the correlation between serum Slit2 protein and the severity of coronary heart disease patients; Logistic regression analysis wasused to analyze the influencing factors of severe coronary lesions in elderly patients with coronary heart disease; receiver operating char-acteristic curve (ROC curve) was used to analyze the predictive power of serum Slit2 protein expression level for severe coronary lesionsin coronary heart disease.Results The levels of total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) in the mild lesion group, moderate lesion group and severe lesion group were significantly higher than those in the control group, andthe level of LDL-C increased with the severity of the lesion (P<0.05). The serum Slit2 protein expression level of the study group [(1.94±0.45)μg/L] was significantly lower than that of the control group [(3.52±0.80)μg/L] (P<0.05). The serum Slit2 protein expression level ofpatients with mild [(2.47±0.55)μg/L], moderate [(1.85±0.41)μg/L] and severe disease group [(1.20±0.34) μg/L] was decreased succes-sively (P<0.05). The Spearman analysis showed that the expression level of Slit2 protein in serum of patients with coronary heart dis-ease was negatively correlated with the severity of the disease (r=.0.53, P<0.05). Multivariate Logistic regression analysis showed that sserum Slit2 protein and LDL-C were the influencing factors of severe coronary lesions in elderly patients with coronary heart disease (P <0.05). The area under ROC curve (AUC) of serum Slit2 was 0.93, the sensitivity was 93.75%, the specificity was 88.37%, the trunca-tion value was 1.54 μg/L, and the Joaden index was 0.82. The AUC of serum LDL-C was 0.89, the sensitivity was 87.50%, the specifici-ty was 86.05%, the truncation value was 3.48 mmol/L, and the Joaden index was 0.74. The AUC of the two combined was 0.96, the sen-sitivity was 96.87%, the specificity was 88.37%, and the Joaden index was 0.85. Slit2 alone predicted a slightly higher AUC than LDL-C alone for severe coronary lesions (P>0.05), but the combination of the two predicted a higher AUC than LDL-C for severe coronary le-sions (Z=2.10, P=0.036), and was more sensitive.Conclusions The expression level of serum Slit2 protein is significantly decreasedin elderly patients with coronary heart disease, and is correlated with the severity of coronary artery disease. Slit2 protein can predictthe severity of coronary artery disease in elderly patients with coronary heart disease, and has high clinical application value.
