      目的探究青年缺血性脑卒中( IS)病人血清微 RNA-218-5p(miR-218-5p)、 LIM和 SH3蛋白 1(LASP1)水平及其应用价值。方法选取 2020年 6月至 2022年 6月山东中医药大学附属医院收治的青年 IS病人 96例为 IS组,对所有 IS病人进行为期 3个月的随访,按照改良 Rankin量表( mRS)评分进行分组,预后良好组 68例( mRS评分 ≤2分)和预后不良组 28例( mRS评分 >2分)。选择同期在该院进行体检的健康志愿者 96例为对照组。血清 miR-218-5p、LASP1 mRNA水平检测采用实时荧光定量 PCR(qRT-PCR); Pearson相关性分析血清 miR-218-5p与 LASP1 mRNA表达水平的关系。采用受试者操作特征曲线( ROC曲线)分析血清中 miR-218-5p、LASP1 mRNA表达水平对 IS预后评估的价值。结果与对照组相比, IS组白细胞计数、总胆固醇、三酰甘油、低密度脂蛋白胆固醇水平显著升高,高密度脂蛋白胆固醇水平显著降低(P<0.05);与对照组(1.03±0.11、1.01±0.11)相比, IS组血清中 miR-218-5p水平 0.88±0.09显著降低, LASP1 mRNA(1.12±0.12)水平显著升高(P<0.05)。IS病人血清 miR-218-5p与 LASP1 mRNA呈负相关( r=.0.73,P<0.001)。与预后良好组( 0.94±0.10、1.05±0.11)相比,预后不良组血清中 miR-218-5p(0.74±0.08)水平显著降低, LASP1 mRNA(1.28±0.13)水平显著升高( P<0.05)。 ROC曲线显示,二者联合评估 IS预后不良的 AUC高于 miR-218-5p、LASP1 mRNA单独预测的 AUC值( Z=12.35,P<0.001;Z=6.60,P=0.010)。结论青年 IS病人血清 miR-218-5p较低, LASP1 mRNA较高,可用于评估青年 IS病人的预后。

      Objective To investigate the application value of the serum microRNA-218-5P (miR-218-5p), LIM and SH3 protein-1 (LASP1) levels in young patients with ischemic stroke (IS). Methods A total of 96 young IS patients admitted to the Affiliated Hospi-tal of Shandong University of Traditional Chinese Medicine from June 2020 to June 2022 were selected as the IS group. All patientswith IS were followed up for 3 months. According to the modified Rankin Scale (mRS) score, 68 patients in the good prognosis group(mRS Score ≤2) and 28 patients in the poor prognosis group (mRS Score > 2) were grouped. A total of 96 healthy volunteers who under-went physical examination in our hospital during the same period were selected as control group. Serum miR-218-5p and LASP1 mRNA levels were detected using real-time fluorescence quantitative PCR (qRT-PCR). The relationship between the level of serum miR-218-5p and LASP1 mRNA was analyzed by Pearson correlation. The predictive value of serum miR-218-5p and LASP1 mRNA ex-pression levels on the prognosis of IS was analyzed by the receiver operating characteristic curve (ROC curve). Results Comparedwith the control group, the levels of white blood cell count, total cholesterol, triglyceride and low density lipoprotein cholesterol in ISgroup were significantly increased, and the levels of high density lipoprotein cholesterol were significantly decreased (P<0.05). Com-pared with the control group (1.03±0.11, 1.01±0.11), the serum miR-218-5p level (0.88±0.09) in IS group was significantly decreased,and the serum LASP1 mRNA level (1.12±0.12) was significantly increased (P<0.05). Serum miR-218-5p was negatively correlated with LASP1 mRNA in IS patients (r=.0.73, P<0.001). Compared with the good prognosis group (0.94±0.10, 1.05±0.11), the serum miR-218-5p (0.74±0.08) level was significantly decreased in the poor prognosis group, and the serum LASP1 mRNA level was significantly in-creased (1.28±0.13) (P<0.05). The ROC curve showed that the AUC of the combination of the two methods was larger than the miR-218-5p and LASP1 mRNA alone (Z=12.35, P<0.001; Z=6.60, P=0.010). Conclusion The serum miR-218-5p was low, and LASP1mRNA was highin young IS patients, and the both were useful for evaluating the prognosis of the young patientswith IS.
