      目的探讨血清锌 α2糖蛋白( ZAG)联合高迁移率族蛋白 B1(HMGB1)对癫痫病人短期内再次发作风险的预测价值。方法选取 2022年 2月至 2023年 2月唐山职业技术学院附属医院癫痫治疗后短期( 1周)内再次发作的 110例全面性发作癫痫病人作为观察组,另选取同期于唐山市协和医院癫痫治疗后短期内未再次发作的 110例全面性发作癫痫病人作为对照组。 Pearson法分析癫痫短期内再次发作病人血清 ZAG和 HMGB1表达水平的相关性, logistic回归分析影响癫痫病人短期内再次发作的相关因素,受试者操作特征曲线( ROC曲线)分析血清 ZAG和 HMGB1对癫痫病人短期内再次发作的预测价值。结果察组血清 ZAG表达水平[( 38.11±4.21)mg/L比( 41.76±5.36)mg/L]低于对照组,血清 HMGB1表达水平[( 6.07±1.63)μg/L比观(4.89±0.92)μg/L]高于对照组( P<0.05);癫痫短期内再次发作病人血清 ZAG、HMGB1表达水平呈显著负相关( r=.0.80,P<0.05);观察组治疗前发作频率 ≥1次/月、多药联合治疗的病人所占比例均高于对照组( P<0.05);治疗前发作频率 ≥1次/月、多药联合治疗、 HMGB1为癫痫病人短期内再次发作的危险因素, ZAG为保护因素( P<0.05);血清 ZAG、HMGB1单独检测预测癫痫病人短期内再次发作的曲线下面积( AUC)分别为[0.73,95%CI:(0.68,0.77); 0.73,95%CI:(0.69,0.77)],二者联合检测的 AUC为 0.79,95%CI:(0.75,0.83)优于血清 ZAG、HMGB1各自单独检测( Z二者联合 -ZAG=3.10、Z二者联合 -HMGB1=2.75,P=0.002、0.005)。结论 血清 ZAG、HMGB1表达水平与,癫痫病人短期内再次发作密切相关,二者联合检测对癫痫病人短期内再次发作具有较高的预测效能。

      Objective To explore the predictive value of serum zinc α2 glycoprotein (ZAG) combined with high mobility group pro-tein B1 (HMGB1) for short-term recurrence risk in epilepsy patients.Methods A total of 110 epileptic patients who experienced short-term (1 week) recurrence after treatment in the Xiehe Hospital of Tangshan from February 2022 to February 2023 were collected as thestudy group, another 110 patients with generalized seizures who did not recur within a short period of time (1 week) after treatment inthe Affiliated Hospital of Tangshan Vocational and Technical College during the same period were selected as the control group. Pear‐son method was applied to analyze the correlation between the expression levels of serum ZAG and HMGB1 in patients with short-term recurrent epilepsy, logistic regression to analyze the relevant factors affecting short-term recurrence in epilepsy patients, and receiveroperating characteristic (ROC) curve to analyze the predictive value of serum ZAG and HMGB1 for short-term recurrence of epilepsy patients.Results The expression level of serum ZAG [(38.11±4.21) mg/L vs. (41.76±5.36) mg/L] in the study group was lower thanthat in the control group, while the expression level of serum HMGB1 [(6.07±1.63)μg/L vs. (4.89±0.92)μg/L] was higher than that in the control group (P<0.05). The expression levels of serum ZAG and HMGB1 in patients with short-term recurrent epilepsy showed an obvi-ous negative correlation (r=.0.80, P<0.05). The proportions of patients with seizure frequency ≥ 1 time/month before treatment and pa-tients receiving multi-drug combination therapy in the study group were higher than those in the control group (P<0.05). Seizure fre-quency ≥ 1 time/month before treatment, multi-drug combination therapy, and HMGB1 were risk factors for short-term recurrence in ep-ilepsy patients, while ZAG was a protective factor (P<0.05). The areas under the curve (AUC) of serum ZAG and HMGB1 for predicting short-term recurrence in epilepsy patients were [0.73, 95%CI:(0.68,0.77) and 0.73, 95%CI:(0.69,0.77)], respectively,and the AUC of the combined detection of the two was 0.79, 95%CI:(0.75,0.83), which was superior to that of the individual detection of serum ZAG and HMGB1 (Zcombination-ZAG=3.10, Zcombination-HMGB1=2.75, P=0.002, 0.005).Conclusion The expression levels of serum ZAG and HMGB1 are closely related to short-term recurrence in epilepsy patients, and the combined detection of the two has higher predictive value.
