      目的观察微波热凝及局部照射治疗对喉结核抗结核治疗效果的影响。方法选取 2018年 10月至 2022年 9月河北省胸科医院的喉结核病人 84例,分为对照组与观察组各 42例,对照组采用规范化 2HRZE/4HR抗结核方案治疗,观察组在对照组治疗的基础上加用微波热凝及局部照射治疗,两组均治疗 6个月。观察两组治疗 2、3、6个月的痰菌转阴率及临床症状改善时间,比较两组治疗前、治疗 6个月的炎症指标[γ干扰素(IFN-γ)、肿瘤坏死因子 α(TNF-α)白细胞介素 -2(IL-2)]变化情况,记录两组治疗期间不良反应发生情况。结果观察组治疗 2、3、6个月的痰菌转阴率高于对、照组( 32.50%、55.00%、95.00%比 12.50%、32.50%、77.50%,P<0.05);观察组声音嘶哑[( 9.50±1.26)d比( 10.85±1.69)d]、咽部不适 /疼痛[( 5.88±1.30)d比( 7.73±1.50)d]、咳嗽症状改善时间[( 11.50±2.26)d比( 13.25±2.31)d]短于对照组( P<0.05);治疗 6个月,两组 IFN-γ、TNF-α、IL-2水平均降低,且观察组较对照组低( P<0.05);两组各种不良反应发生率比较,差异无统计学意义( P>0.05)。结论微波热凝及局部照射辅助治疗喉结核能显著缩短病人临床症状改善时间,提高痰菌转阴率,降低机体炎症指标水平,且安全可靠。

      Objective To observe the effect of microwave thermocoagulation and local irradiation on anti-tuberculosis treatment of la-ryngeal tuberculosis.Methods A total of 42 patients who were treated with standardized 2HRZE/4HR anti-tuberculosis regimen atHebei Chest Hospital from October 2018 to September 2022 were selected as the treatment group, and they were treated with micro-wave thermocoagulation and local irradiation. In addition, 42 patients who were treated with standardized 2HRZE/4HR anti-tuberculo-sis regimen alone during the same period were selected as the control group, and both groups were treated for 6 months. The sputumnegative conversion rate and clinical symptom improvement time of the two groups were observed at 2, 3 and 6 months after treatment.The changes of inflammatory indexes [γ-interferon (IFN-γ), tumor necrosis factor-α(TNF-α), interleukin-2 (IL-2)] before treatment and6 months after treatment were compared between the two groups. The incidence of adverse reactions during treatment was recorded inthe two groups.Results The sputum negative conversion rate of the treatment group was higher than that of the control group at 2, 3and 6 months of treatment(32.50%, 55.00%, 95.00% vs. 12.50%, 32.50%, 77.50%, P<0.05). The improvement time of hoarseness [(9.50±1.26)d vs. (10.85±1.69)d], pharyngeal discomfort/pain [(5.88±1.30)d vs. (7.73±1.50)d] and cough symptoms [(11.50±2.26)d vs. (13.25±2.31)d] in the treatment group was shorter than that in the control group (P<0.05). After 6 months of treatment, the levels of IFN-γ, TNF-α, and IL-2 in both groups decreased, and the treatment group was lower than the control group (P<0.05). The incidence of vari-ous adverse reactions in the treatment group was slightly lower than that in the control group, but the difference was not statistically sig-nificant (P>0.05).Conclusion Microwave thermocoagulation and local irradiation in the treatment of laryngeal nuclear energy signifi-cantly shortens the time for clinical symptom improvement in patients, increases sputum negative conversion rate, reduces the level ofinflammatory indicators in the body, and is safe and reliable.
