| 目的探讨结肠癌病人血清微 RNA-20a(miR-20a)、微 RNA-615(miR-615)水平与肿瘤恶性程度的相关性。方法回顾性选取 2021年 8月至 2022年 12月唐山市人民医院收治的 127例结肠癌病人为研究组，另选取同期该院收治的结肠良性疾病病人 122例为对照组。 Pearson法分析结肠癌病人血清 miR-20a、miR-615与癌胚抗原( CEA)及糖类抗原 -199(CA-199)水平的相关性；受试者操作特征曲线( ROC曲线)分析血清 miR-20a、miR-615联合 CEA、CA-199水平对结肠癌的诊断价值。结果与对照组比较，研究组血清 miR-20a(1.19±0.33比 1.06±0.32)、 CEA［( 7.73±2.55)μg/L比( 4.70±1.79)μg/L］和 CA-199［( 14.82±4.01)U/mL比( 9.84±3.21)U/mL］水平较高( P<0.05)，而 miR-615(0.87±0.36比 1.06±0.32)下降( P<0.05)。结肠癌病人 T分期、分化程度、淋巴结转移情况与血清 miR-20a、miR-615表达水平有关( P<0.05)。 Pearson相关性分析显示，结肠癌病人血清 miR-20a与 CEA、CA-199水平呈正相关( P<0.05)血清 miR-615水平与 CEA、CA-199水平呈负相关( P<0.05)。血清 miR-20a、miR-615、CEA和 CA-199水平单独诊断结肠癌发生的，曲线下面积( AUC)分别为 0.82［95%CI:(0.77，0.87)］、 0.79［95%CI:(0.74，0.84)］、 0.84［95%CI:(0.78，0.88)］和 0.84［95%CI:(0.78，0.88)］；四个指标联合诊断的 AUC［0.96，95%CI:(0.93，0.98)］大于血清 miR-20a(Z=5.44，P<0.001)、 miR-615(Z=5.68，P<0.001)、 CEA(Z=5.62，P<0.001)和 CA-199(Z=5.37，P<0.001)水平单独诊断的 AUC。结论结肠癌病人血清中 miR-20a表达升高、 miR-615表达降低，二者联合 CEA和 CA-199对结肠癌的诊断价值较高。 |

| Objective To investigate the correlation between serum microRNA-20a (miR-20a) and microRNA-615 (miR-615) levels and the malignant degree of colon cancer patients.Methods A retrospective study was conducted on 127 colon cancer patients ad.mitted to Tangshan People's Hospital from August 2021 to December 2022, and an additional 122 patients with benign colon diseas.es admitted to the same hospital during the same period were regarded as the benign control group. The correlation between serummiR-20a, miR-615 and carcinoembryonic antigen (CEA) and carbohydrate antigen-199 (CA-199) levels in patients with colon cancerwas analyzed by Pearson correlation coefficient. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic val. ue of serum miR-20a, miR-615 combined with CEA and CA-199 levels in colon cancer.Results Compared with the benign control group, the study group had higher levels of serum miR-20a (1.19±0.33 vs. 1.06±0.32), CEA [(7.73±2.55) μg/L vs. (4.70±1.79) μg/L], and CA-199 [(14.82±4.01) U/mL vs. (9.84±3.21) U/mL](P<0.05), but lower miR-615 (0.87±0.36 vs. 1.06±0.32) (P<0.05). The expres. sion levels of miR-20a and miR-615 in serum were correlated with T stage, differentiation degree and lymph node metastasis in pa. tients with colon cancer (P<0.05). Pearson correlation analysis showed that serum miR-20a was positively correlated with CEA and CA-199 levels (P<0.05), while serum miR-615 was negatively correlated with CEA and CA-199 levels (P<0.05). The areas under the curve (AUC) of serum miR-20a, miR-615, CEA and CA-199 for the diagnosis of colon cancer alone were 0.82 [95%CI:(0.77, 0.87)], 0.79[95%CI: (0.74,0.84)], 0.84 [95%CI: (0.78,0.88)] and 0.84 [95%CI: (0.78, 0.88)]; The AUC of the four indicators combined for the diagnosis [0.96, 95%CI:(0.93, 0.98)] was higher than that of serum miR-20a (Z=5.44, P<0.001), miR-615 (Z=5.68, P<0.001), CEA (Z =5.62, P<0.001) or CA-199 (Z=5.37, P<0.001) alone.Conclusions The expression level of miR-20a is increased and that of miR-615 is decreased in the serum of colon cancer patients. Their combination with CEA and CA-199 has high diagnostic value for colon cancer. |
