| 目的探讨自拟益气养阴活血通络方联合针灸治疗气阴两虚兼瘀型 2型糖尿病合并勃起功能障碍( T2DM合并 ED)疗效及对勃起质量量表( EQS)评分、勃起硬度( EHS)评分的影响。方法选取 2019年 6月至 2020年 12月唐山市中医医院气阴两虚兼瘀型 T2DM合并 ED的男性病人 120例进行前瞻性研究，按随机数字表法分为 A组( 40例)、 B组( 40例)和 C组( 40例)。 A组给予西医基础治疗； B组在 A组治疗的基础上联合自拟益气养阴活血通络方治疗； C组在 B组治疗的基础上配合针灸治疗。比较三组治疗前后国际勃起功能指数 -5(IIEF-15)评分、中医证候积分、 EQS评分、 EHS评分、阴茎血流动力学［阴茎血管收缩期峰值流速( PSV)舒张期峰值流速(EDV)和阻力指数(RI)］及血清性激素［睾酮( T)、雌二醇( E2)］、血管内皮功能［内皮素 -1(ET-1)、一氧化氮(NO)、、血管紧张素 Ⅱ(AngⅡ)］水平。观察三组临床疗效及不良反应情况。结果 C组总体疗效(治愈 18例、显效 15例、有效 5例、无效 2例)均优于 A组(治愈 8例、显效 13例、有效 7例、无效 12例)、 B组(治愈 9例、显效 15例、有效 8例、无 8效例)(P<0.05)。治疗后 C组、 B组中医证候积分均低于 A组，且 C组中医证候积分低于 B组( P<0.05)。治疗后 C组、 B组 IIEF-5评分、 EQS评分、 EHS评分均高于 A组，且 C组 IIEF-5评分、 EQS评分、 EHS评分高于 B组( P<0.05)。治疗后 C组、 B组 T水平均高于 A组，且 C组 T水平高于 B组( P<0.05)；治疗后 C组、 B组 E2水平均低于 A组，且 C组 E2水平低于 B组( P<0.05)。治疗后 C组、 B组 PSV、RI水平高于 A组，且 C组 PSV、RI水平高于 B组( P<0.05)；治疗后 C组 EDV均低于 B组、 A组( P<0.05)，但 B组、 A组比较差异无统计学意义( P>0.05)。治疗后 C组、 B组 AngⅡ水平低于 A组，且 C组 AngⅡ水平低于 B组(P<0.05)；治疗后 C组 ET-1水平均低于 B组、 A组( P<0.05)但 B组、 A组比较差异无统计学意义( P>0.05)；治疗后 C组、 B组 NO水平高于 A组，且 C组 NO水平高于 B组( P<0.05)。结论自，拟益气养阴活血通络方联合针灸治疗气阴两虚兼瘀型 T2DM合并 ED病人，疗效确 |

| Objective To investigate the curative effect of self-made Yiqi Yangyin Huoxue Tongluo recipe combined with acupunc-ture on type 2 diabetes mellitus complicated with erectile dysfunction (T2DM and ED) caused by qi yin deficiency and blood stasis, and its influence on the score of erectile quality scale (EQS) and erectile hardness score (EHS).Methods One hundred and twenty male patients with T2DM and ED of type of deficiency of both qi and yin with blood stasis in Tangshan Traditional Chinese Medicine Hospi-tal from June 2019 to December 2020 were selected for prospective study. They were assigned into group A (40 cases), group B (40 cas-es) and group C (40 cases) according to random number table method. Group A was given basic Western medicine treatment; Group Bwas treated on the basis of group A combined with self-prepared Yiqi Yangyin Huoxue Tongluo prescription; Group C was treated with acupuncture and moxibustion on the basis of group B. The international erectile function index -5 (IIEF-15) scores, traditional chinesemedicine (TCM) syndrome scores, EQS scores, EHS scores, penis hemodynamics [peak systolic velocity (PSV), peak diastolic velocity(EDV) and resistance index (RI)], the levels of serum sex hormones [testosterone (T), estradiol (E2)] and vascular endothelial function[endothelin -1 (ET-1), nitric oxide (NO) and angiotensin Ⅱ (Ang Ⅱ)] were compared among the three groups before and after treatment. The clinical efficacy and adverse reactions of the three groups were observed.Results The overall efficacy of group C (cured 18 cases,effective 15 cases, effective 5 cases, ineffective 2 cases) was better than that of group A (cured 8 cases, effective 13 cases, effective 7cases, ineffective 12 cases) and group B (cured 9 cases, obvious 15 cases, effective 8 cases, no effect 8 cases) (P<0.05). After treatment,the TCM syndrome score of group C and group B was lower than that of group A, and the TCM syndrome score of group C was lowerthan that of group B (P<0.05).After treatment, IIEF-5 scores,EQS scores and EHS scores of groups C and B were higher than those of group A, and IIEF-5 scores,EQS scores and EHS scores of group C were higher than those of group B (P<0.05). After treatment,the Tlevel in group C and group B was higher than that in group A,and the T level in group C was higher than that in group B(P<0.05).After treatment, E2level in group C and group B was lower than that in group A,and E2level in group C was lower than that in group B(P< 0.05). After treatment,PSV and RI levels in groups C and B were higher than those in group A, and PSV and RI levels in group C werehigher than those in group B(P<0.05).After treatment, EDV in group C was lower than that in group B and Group A(P<0.05),but there was no significant difference between group B and group A(P>0.05).After treatment,AngⅡlevel in group C and group B was lower thanthat in group A,and AngⅡ level in group C was lower than that in group B(P<0.05).After treatment, ET-1 level in group C was lower than that in group B and group A(P<0.05),but there was no statistical significance between group B and group A(P>0.05).After treat-ment,the NO level in group C and group B was higher than that in group A,and the NO level in group C was higher than that in group B(P<0.05).Conclusion Self-made Yiqi Yangyin Huoxue Tongluo recipe combined with acupuncture has definite curative effect on T2DMand ED with type of deficiency of both qi and yin with blood stasis,which can significantly improve TCM syndrome integral, improve erec-tile function,quality and hardness,adjust sex hormone level,improve penis vascular function and hemodynamics,and has good safety. |
