目的分析验证多发性骨髓瘤( MM)和弥漫大 B细胞淋巴瘤( DLBCL)是否存在继发关系,指导相应的化疗方案。方法回顾性分析 2023年 4月 21日胜利油田中心医院收治的 1例 MM合并 DLBCL的病人,应用二代测序( NGS)技术,并对其基因序列进行分析。结果对该病例的肠道病灶及胸壁肿物进行免疫球蛋白重链可变区的检测结果提示两者为独立发病。前后给予病人 4个疗程 “T-VBD方案”和 4个疗程 “VRD方案”,疗效评价为完全缓解。结论 MM合并 DLBCL临床罕见,可应用 NGS技术进行基因分析,验证两病发病顺序,实现个体化精准医疗。 |

Objective To analyze and verify whether there is a secondary relationship between multiple myeloma (MM) and diffuselarge B cell lymphoma (DLBCL), and to provide guidance for the corresponding chemotherapy regimens. Methods A retrospectiveanalysis was made of a patient with MM complicated by DLBCL, who was admitted to Shengli Oilfield Central Hospital on April 21,2023. The next-generation DNA sequencing (NGS) technology was applied and an analysis was made of the gene sequence of this pa-tient.Results The results of immunoglobulin heavy chain variable region testing of the intestinal lesion and chest wall mass suggestedthat they were independent. After 4 courses of "T-VBD regimen" and 4 courses of "VRD regimen", the patient achieved complete re-mission.Conclusion Co-occurrence of MM and DLBCL is clinically rare, and the next-generation DNA sequencing technology can beapplied for genetic analysis to verify the order of onset of the two diseases and to guide individualized precision treatment. |
