      目的探讨非酒精性脂肪性肝病( NAFLD)病人肠道菌群生态特征与血清甲胎蛋白异质体 3(AFP-L3)、白细胞介素 -19(IL-19)水平的相关性。方法选择 2020年 6月到 2022年 6月在郑州市第一人民医院接受治疗的 NAFLD病人 106例作为 NAFLD组,同期选取体检健康志愿者 106例作为健康组。采用酶联免疫吸附试验( ELISA)测定血清 IL-19、AFP-L3水平;采用 Pearson相关性分析检验肠道菌群生态特征与血清 AFP-L3、IL-19水平之间的相关性。结果 NAFLD组的谷草转氨酶、谷丙转氨酶、空腹血糖、尿酸、血尿素氮、血清肌酐、总胆固醇( TC)、甘油三酯( TG)均高于健康组,且差异有统计学意义( P<0.05)。 NAFLD组的 AFP-L3水平[(94.43±16.18)μg/L]高于健康组[(76.02±12.12)μg/L]NAFLD组的 IL-19水平[(47.23±8.05)ng/L]低于健康组[( 65.82±12.16)ng/L],差异有统计学意义( P<0.05)。 NAFLD组乳酸杆,菌、拟杆菌、双歧杆菌丰度少于健康组( P<0.05)粪肠球菌、屎肠球菌、大肠杆菌丰度多于健康组(P<0.05)。根据 Pearson相关性分析可以得知,乳酸杆菌、拟杆菌、双歧杆菌丰AFP-L3水平呈负相关( P<0.05)与 IL-19水平呈正相关( P<0.05);粪肠球菌、屎肠球菌、大肠杆菌丰度与 AFP-L3水平呈正相关( P<0.05),与 IL-19水平呈负相关(P<0.05)。结论肠道菌群生态特征与 NAFLD病人血清中 AFP-L3、IL-19水平密切度与相关。

      Objective To explore the correlation between gut microbiota ecological characteristics and serum alpha-fetoprotein iso-form 3 (AFP-L3) and interleukin-19 (IL-19) expression levels in patients with non-alcoholic fatty liver disease (NAFLD). Methods From June 2020 to June 2022, 106 NAFLD patients treated at Zhengzhou First People's Hospital were enrolled in the NAFLD group,and 106 healthy volunteers undergoing physical examinations served as the health group. Serum IL-19 and AFP-L3 levels were mea-sured via enzyme-linked immunosorbent assay (ELISA). Pearson correlation analysis was used to evaluate relationships between gut mi-crobiota ecological characteristics and serum AFP-L3 and IL-19 levels. Results Compared to the health group, the NAFLD groupshowed significantly higher levels of aspartate aminotransferase, alanine aminotransferase, fasting blood glucose, uric acid, blood ureanitrogen, serum creatinine, total cholesterol, and triglycerides (P<0.05). The NAFLD group exhibited significantly higher AFP-L3 lev-els (94.43±16.18) μg/L lower IL-19 levels (47.23±8.05) ng/L compared to the health group [(76.02±12.12) μg/L for AFP-L3; (65.82± 12.16) ng/L for IL-19], with statistical significance differences (P<0.05). The abundance of Lactobacillus, Bacteroides, and Bifidobacteri. um was lower in the NAFLD group than in health group, whereas Enterococcus faecalis, Enterococcus faecalis, and Escherichia coli were more abundant (P<0.05). Pearson correlation analysis showed that Lactobacillus, Bacteroides, and Bifidobacterium abundance was nega-tively correlated with AFP-L3 levels (P<0.05) and positively correlated with IL-19 level (P<0.05). Conversely, Enterococcus faecalis, En. terococcus faecalis, and Escherichia coli was positively correlated with AFP-L3 levels (P<0.05) and negatively correlated with IL-19 lev-el (P<0.05). Conclusion Gut microbiota ecological characteristics are closely associated with serum AFP-L3 and IL-19 levels in NAFLD patients.
