肿瘤相关成纤维细胞(CAFs)是肿瘤微环境中的关键组成部分,广泛参与肿瘤的增殖、侵袭、转移及免疫逃逸等过程。本文综述了CAFs在肿瘤放射治疗中的作用及其机制。研究发现,CAFs具有较强的放射抗性,放疗虽可引起其DNA损伤,但并未完全影响其对肿瘤的生物活性。辐射可促使CAFs释放多种细胞因子,如TGF-β、VEGF、IL-6等,从而改变肿瘤微环境,增强肿瘤细胞的放疗耐受性。CAF的活化可能通过诱导上皮-间质转化与促进肿瘤干性增强,进一步加剧复发与转移。不同CAF亚型在结构重塑及免疫调节中具有不同功能,也对放疗效果产生差异影响。目前,多种CAF靶向抑制剂在体内外实验及早期临床研究中展现良好抗肿瘤潜力,联合放射治疗有广阔前景。 |

Cancer-associated fibroblasts (CAFs) are pivotal components of the tumor microenvironment and play critical roles in promoting tumor proliferation, invasion, metastasis, and immune evasion. This review summarizes the roles and underlying mechanisms of CAFs in tumor radiotherapy. Studies have shown that CAFs exhibit strong radioresistance; although radiotherapy induces DNA damage in CAFs, it does not fully impair their tumor-promoting functions. Radiation can stimulate CAFs to secrete a variety of cytokines, such as TGF-β, VEGF, and IL-6, thereby modulating the tumor microenvironment and enhancing the radioresistance of tumor cells. Activation of CAFs may induce epithelial-mesenchymal transition and enhance tumor stemness, further promoting recurrence and metastasis. Distinct CAF subtypes contribute differently to structural remodeling and immune regulation,leading to heterogeneous radiotherapeutic outcomes. Several CAF-targeting inhibitors have shown promising anti-tumor activity in preclinical and early-phase clinical studies. Their combination with radiotherapy offers a potential strategy to improve therapeutic efficacy in cancer treatment. |
