      目的探讨慢性自发性荨麻疹( CSU)病人血清人 β防御素 2(hBD-2)、嗜酸性粒细胞衍生神经毒素( EDN)水平变化及其与病情的相关性。方法武汉市第六医院 2019年 1月至 2021年 1月收治的 119例 CSU病人和体检的 50例健康者分别纳入试验组和对照组,试验组根据荨麻疹活动评分( UAS)将病人分为轻、中、重度组( 51、38、30例)。对轻度组、中度组和重度组的一般资料进行比较;采用酶联免疫吸附试验( ELISA)对各组血清 hBD-2、EDN水平及 UAS进行检测,采用 Pearson相关性分析、 logistic回归分析分别对病人血清 hBD-2、EDN水平与 UAS相关性进行分析、探索影响病人病情严重程度的相关因素。结果轻度组、中度组、重度组的性别构成、年龄、病程、是否存在过敏史比较,均差异无统计学意义( P>0.05),是否存在感染史、 UAS比较,均差异有统计学意义( P<0.05);试验组血清 hBD-2[(122.21±26.24)ng/L]、 EDN[(201.21±35.92)μg/L]水平均高于对照组[( 102.81±15.27)ng/L、(95.38±12.57)μg/L](P<0.05);且重度组血清 hBD-2[(139.82±23.01)ng/L]、 EDN[(257.92±39.76)μg/L]水平显著高于轻度组[(111.05±25.61)ng/L、(156.38±29.67)μg/L]和中度组[(123.28±29.63)ng/L、(216.59±41.28)μg/L],均差异有统计学意义( P<0.05); Pearson相关性分析显示, CSU病人血清 hBD-2、EDN水平与 UAS呈正相关( r=0.41、0.63,均 P<0.001); logistic回归分析显示, UAS、血清 hBD-2、EDN水平是 CSU病人病情严重程度的危险因素(P<0.05)。结论 CSU病人血清 hBD-2、 EDN水平与其病情严重程度密切相关,检测病人血清 hBD-2、EDN水平可评估病人病情严重程度,并采取有效的干预措施延缓病情的进展。

      Objective To investigate variations in serum levels of human beta-defensin 2 (hBD-2) and eosinophil-derived neurotoxin (EDN) in patients with chronic spontaneous urticaria (CSU) and their association with disease severity.Methods A total of 119 pa-tients treated at Wuhan Sixth Hospital from January 2019 to January 2021 and 50 healthy controls from routine physical examinationswere enrolled as the CSU group and control group, respectively. The CSU group was classified into mild (51 cases), moderate (38 cases),and severe (30 cases) subgroups based on Urticaria Activity Score (UAS). Demographic and clinical characteristics (gender, age, dis-ease duration, allergy history, infection history) were compared across subgroups. Serum hBD-2 and EDN levels were measured via en-zyme-linked immunosorbent assay (ELISA). Correlations between biomarker levels and UAS were analyzed using Pearson's test; logis-tic regression identified risk factors for disease severityResults No significant differences were observed in gender distribution, age,disease duration, or allergy history among the mild, moderate, and severe groups (all P>0.05). Significant differences were found in in-fection history and UAS (both P<0.05). The experimental group had higher serum hBD-2 levels [(122.21±26.24) ng/L vs. (102.81± 15.27) ng/L] and EDN levels [(201.21±35.92) μg/L vs. (95.38±12.57) μg/L] compared to the control group (both P<0.05). The severe group exhibited significantly higher hBD-2 levels [(139.82±23.01) ng/L] than the mild group [(111.05±25.61) ng/L] and moderate group[(123.28±29.63) ng/L], and higher EDN levels [(257.92±39.76) μg/L] than the mild group [(156.38±29.67) μg/L] and moderate group[(216.59±41.28) μg/L] (all P<0.05). Pearson correlation analysis revealed positive correlations between serum hBD-2(r=0.41) and EDN (r=0.63) levels with UAS (both P<0.001). Logistic regression analysis identified UAS, serum hBD-2, and EDN levels as risk fac-tors for disease severity (all P<0.05).Conclusions Serum hBD-2 and EDN levels in CSU patients are closely associated with diseaseseverity. Measuring these biomarkers may aid in assessing disease progression and guiding clinical interventions.
