目的比较角膜塑形( OK)镜、多点离焦眼镜( DIMS)在轻度近视青少年中的应用效果。方法收集 2021年 12月至 2022年 12月在北京新世纪儿童医院就诊的 95例( 183眼)轻度近视青少年,配戴 OK镜的 49例( 94眼)近视青少年为 OK镜组,配戴 DIMS的 46例( 89眼)近视青少年为 DIMS组。比较配戴前、配戴 3个月、配戴 6个月、配戴 12个月两组青少年等效球镜度(SE)及 SE变化量、眼轴长度( AL)及 AL增长量、中央角膜厚度( CCT)、角膜内皮细胞密度( CECD)、泪膜破裂时间( BUT)变化,统计两组并发症发生情况。结果配戴 12个月两组 SE值[(.2.28±0.68)D、(.2.31±0.70)D]与配戴前[(.1.83±0.58)D、(.1.81± 0.54)D]比较差异有统计学意义(P<0.05);但配戴 12个月 OK镜组、 DIMS组 SE值以及 12个月 SE变化量比较差异无统计学意义(P>0.05)。配戴 6个月( 24.43±0.26)mm、12个月 DIMS组 AL值( 24.60±0.30)mm长于配戴前( 24.27±0.27)mm,配戴 12个月 OK镜组 AL值( 24.55±0.28)mm长于配戴前( 24.29±0.26)mm(P<0.05);配戴 12个月 OK镜组 AL值(24.55±0.28)mm、12个月 AL增长量( 0.28±0.10)mm数值优于 DIMS组[(24.60±0.30)mm、(0.31±0.13)mm]。配戴 6个月( 9.62±1.30)s、12个月 OK镜组 BUT(8.75±1.23)s短于 DIMS组[( 12.01±1.71)s、(11.87±1.75)s](P<0.05)。 OK镜组并发症发生率 9.57%高于 DIMS组 2.25%(P<0.05)。结论 OK镜、 DIMS应用于轻度近视青少年,均可延缓青少年近视加深,两者均未对角膜指标产生明显影响, OK镜 AL抑制程度优于 DIMS,但会缩短 BUT,而 DIMS安全性更佳。 |

Objective To compare the effects of orthokeratology (OK lens) and defocus incorporated multiple segments (DIMS) inthe juveniles with mild myopia.Methods Ninety-five cases (183 eyes) of mild myopia adolescents were collected from December2021 to December 2022 in Beijing New Century Children's Hospital. Forty-nine cases (94 eyes) of myopic adolescents wearing OKlens were included in the OK lens group, while 46 cases (89 eyes) of myopic adolescents wearing DIMS were included in the DIMSgroup. The changes of spherical equivalent (SE), SE change, axial length (AL), AL increase, central corneal thickness (CCT), cornealendothelial cell density (CECD) and tear break-up time (BUT) were compared before wearing, and 3 months, 6 months, 12 months af-ter wearing, and the incidence of complications in the two groups were statistically analyzed. Results SE value between the two groups for 12 months after wearing [(.2.28±0.68) D, (.2.31±0.70) D] were significantly different from those before wearing [(.1.83±0.58) D, (.1.81±0.54) D] (P < 0.05). However, there was no significant difference in SE value and 12-month SE change between 12-month OK lens group and DIMS group (P > 0.05). The AL values in the 6-month and 12-month DIMS groups were longer than that be-fore wearing [(24.43±0.26) mm vs. (24.60±0.30) mm vs. (24.27±0.27) mm], and the AL value in the 12-month OK lens group was lon-ger than that before wearing [(24.55±0.28) mm vs. (24.29±0.26) mm] (P < 0.05). AL value and AL growth in 12-month OK lens group were better than those in DIMS group [(24.55±0.28) mm vs. (24.60±0.30)mm, (0.28±0.10) mm vs. (0.31±0.13) mm]. BUT in 6-month and 12-month OK lens group and 12 months was shorter than that in DIMS group [(9.62±1.30) s vs. (12.01±1.71) s, (8.75±1.23) s vs. (11.87±1.75) s] (P<0.05). The complication rate in the OK lens group was higher than that in the DIMS group (9.57% vs. 2.25%) (P< 0.05).Conclusions Both OK lens and DIMS can delay the progression of myopia in adolescents with mild myopia, and neither of them has a significant impact on corneal indexes, but the AL inhibition degree of OK lens is better than that of DIMS. Moreover, OK lens willshorten BUT, yet DIMS is safer. |
