| 目的观察三黄止痒洗剂联合曲安奈德益康唑乳膏治疗急性肛周湿疹( APE)的效果。方法回顾性分析 2021年 4月至 2023年 4月河北省沧州中西医结合医院收治的 APE病人 120例的临床资料，采用 1∶1倾向性评分匹配的方法控制混杂偏倚，匹配后，对照组 60例予曲安奈德益康唑乳膏，研究组 60例予三黄止痒洗剂联合曲安奈德益康唑乳膏。比较两组的疗效、皮损状况、肛门局部微循环、外周血 Th1/Th2相关因子、复发率。结果研究组的总有效率高于对照组( P<0.05)。治疗后，两组的肛门瘙痒评分、湿疹面积评分、皮疹形态评分、肛周渗液评分、白细胞介素( IL)-4均低于治疗前( P<0.05)且研究组低于对照 |

| Objective To observe the efficacy of Sanhuang antipruritic lotion combined with triamcinolone acetonide and econazole nitrate cream for acute perianal eczema (APE).Methods A retrospective analysis was conducted on the clinical data of 120 patientswith APE admitted to Cangzhou Hospital of Integrated TCM-WM·Hebei from April 2021 to April 2023, and a 1∶1 propensity scorematching method was used to control for confounding bias. After matching, 60 cases in the control group received triamcinolone aceton-ide and econazole nitrate cream, while 60 cases in the study group received Sanhuang antipruritic lotion combined with triamcinoloneacetonide and econazole nitrate cream. The efficacy, skin lesion status, local anal microcirculation, peripheral blood Th1/Th2 related fac-tors, and recurrence rate were compared.Results The total effective rate of the study group was higher than that of the control group (P< 0.05). After treatment, the scores of anal itching, eczema area, rash morphology, perianal effusion, and interleukin-4 of both groups were lower than those before treatment (P<0.05), with the study group showing lower scores than the control group (P<0.05). After treatment, the wound blood flow, transcutaneous partial pressure of oxygen, interferon-γ, and IL-2 in both groups were higher than those before treatment (P<0.05), with the study group showing higher values than the control group (P<0.05). The recurrence rate in the study group (8.77%, 5/57) was lower than that in the control group (25.00%, 12/48) (P=0.025).Conclusion The combination of Sanhuang antipru-ritic lotion and triamcinolone acetonide and econazole nitrate cream has a definite therapeutic effect on APE, which can improve the con-dition of skin lesions, promote local anal microcirculation, regulate peripheral blood Th1/Th2 related factors, and reduce recurrence rate. |
