      目的探究血清几丁质酶 -3样蛋白 1(CHI3L1)、前梯度蛋白 2(AGR2)联合多参数磁共振成像( mp-MRI)在前列腺癌诊断中的临床价值。方法回顾性分析选取 2022年 7月至 2023年 6月邯郸市第一医院收治的经影像学以及病理检测确诊的前列腺癌病人 122例为前列腺癌组, 120例前列腺增生病人为对照组。所有研究对象在入院次日清晨进行 mp-MRI检测,根据前列腺影像报告和数据系统第 2版( PI-RADS v2)评分标准进行评分。使用酶联免疫吸附分析( ELISA)检测血清中 CHI3L1、 AGR2水平。受试者操作特征曲线( ROC曲线)分析血清 CHI3L1、AGR2联合 mp-MRI对前列腺癌的诊断价值。结果与对照组[( 5.98±0.65)μg/L、(13.68±1.45)μg/L]相比,前列腺癌组病人血清中 CHI3L1、AGR2水平[( 6.85±0.75)μg/L、(15.67±1.68)μg/L]显著升高( P<0.05)。前列腺癌组和对照组 PI-RADS v2评分组间比较差异有统计学意义( P<0.05)前列腺癌组 4、5分的病人显著高于对照组( P<0.05)。血清 CHI3L1、AGR2联合 mp-MRI诊断前列腺癌的曲线下面积均大于单一指标诊断( Z=8.06,P<0.001;Z=9.73,P<0.001;Z=5.83,P<0.001)。 CHI3L1、AGR2表达水平与前列腺癌病人 TNM临床分期有关( P<0.05)。结论前列腺癌病人血清中 CHI3L1、AGR2水平显著升高,二者联合 mp-MRI能够提高前列腺癌的诊断价值。

      Objective To investigate the clinical value of serum chitinase-3 like protein 1 (CHI3L1) and pre gradient protein 2 (AGR2) combined with multiparameter magnetic resonance imaging (mp-MRI) in the diagnosis of prostate cancer.Methods A retro-spective analysis of 122 patients with prostate cancer diagnosed by imaging and pathological examination in the First Hospital of Han-dan from July 2022 to June 2023 was selected as the prostate cancer group, and 120 patients with prostate hyperplasia were selected asthe control group. All study subjects underwent mp-MRI testing on the morning of admission and were scored according to the prostate Imaging reporting and Data system version 2 (PI-RADS v2) scoring standard. Enzyme linked immunosorbent assay (ELISA) was appliedto detect the levels of CHI3L1 and AGR2 in serum. Receiver operating characteristic (ROC) curve was applied to analyze the diagnosticvalue of serum CHI3L1, AGR2, and mp-MRI for prostate cancer.Results Compared with the control group [(5.98±0.65) μg/L, (13.68±1.45) μg/L], the serum levels of CHI3L1 and AGR2 in the prostate cancer group [(6.85±0.75) μg/L, (15.67±1.68) μg/L] were signifi-cantly increased (P<0.05). There was a obvious difference in PI-RADS v2 score between the prostate cancer group and the control group (P<0.05), the patients with 4 scores and 5 scores in the prostate cancer group were obviously higher than those in the control group (P<0.05). The area under the curve of serum CHI3L1, AGR2 combined with mp-MRI in the diagnosis of prostate cancer was greater than that of single indicator diagnosis (Z=8.06, P<0.001; Z=9.73 P<0.001; Z=5.83, P<0.001). The expression levels of CHI3L1 and AGR2 were associated with TNM clinical staging in prostate cancer patients (P<0.05).Conclusion The serum levels of CHI3L1 and AGR2 in prostate cancer patients are significantly elevated, and the combination of the two can improve the diagnostic value ofprostate cancer.
