      目的探讨癫痫病人预期性悲伤现状,并分析其影响因素。方法采用便利抽样法,选取 2021年 2月至 2023年 7月收治四川大学华西医院的 150例癫痫病人作为研究对象。选用自制调查问卷了解病人人口社会学资料、临床资料、预期性悲伤、家庭关怀度、社会支持度及疾病感知情况,采用多重线性回归分析癫痫病人预期性悲伤的影响因素,采用分层线性模型分析不同层面影响因子对癫痫病人预期性悲伤的影响。结果此次研究共发放 150份调查问卷,收回有效问卷 145份。 145例癫痫病人预期性悲伤量表( AGS)评分为( 35.26±5.07)分,处于中等水平。 Pearson相关系数分析结果显示: AGS评分与家庭关怀度指数问卷( APGAR)评分呈负相关( r=.0.53,P<0.001)与社会支持评定量表( SSRS)评分呈负相关( r=.0.47,P<0.001)。不同性别、职业状态、癫痫病程、近 1个月内发作次数、服药规律,性的癫痫病人 AGS总分对比,差异有统计学意义( P<0.05)。多重线性回归分析结果显示:性别、职业状态、近 1个月发作次数及 APGAR评分是癫痫病人预期性悲伤的影响因素( P<0.05)。分层线性模型分析显示:性别、 APGAR评分对癫痫病人预期性悲伤有显著预测价值( P<0.05);在跨层级交互作用中, APGAR评分与性别的交互项对癫痫病人预期性悲伤的影响显著( P<0.001)。结论癫痫病人预期性悲伤处于中等水平,与家庭关怀度及社会支持度呈负相关;性别、职业状态、近 1个月发作次数及家庭关怀度是其影响因素;其中性别和家庭关怀程度对癫痫病人预期性悲伤有预测价值,并且家庭关怀程度缓解了性别与预期性悲伤的正向关系。因此临床医护人员可从提高家庭关怀度的角度出发,采取有效的干预措施,以改善预期性悲伤反应。

      Objective To explore the current situation of anticipatory grief among patients with epilepsy and analyze its influencing factors. Methods Using the convenience sampling method, 150 patients with epilepsy admitted to West China Hospital of SichuanUniversity from February 2021 to July 2023 were selected as research subjects. A researcher-designed questionnaire was used to col-lect data on patients' demographic and sociological data, clinical information, clinical data, anticipatory grief, family care (assessed bythe Family APGAR Index), social support, and disease perception. Multiple linear regression was applied to analyze the influencing fac-tors of anticipatory grief in patients with epilepsy, and a hierarchical linear model was used to analyze the impact of influencing factorsat different levels on anticipatory grief.Results A total of 150 questionnaires were distributed, and 145 valid questionnaires were col-lected. The Anticipatory Grief Scale (AGS) score of the 145 epileptic patients was (35.26±5.07) points, indicating a moderate level ofanticipatory grief. Pearson correlation coefficient analysis showed that the AGS score was negatively correlated with the Family APGARIndex (APGAR) score (r = .0.53, P < 0.001) and negatively correlated with Social Support Rating Scale (SSRS) score (r = .0.47, P < 0.001). There were statistically significant differences in the total AGS scores among patients with epilepsy of different gender, occupa-tional status, epilepsy durations, seizures frequencies in the past month, and medication adherence (P < 0.05). Multiple linear regres-sion analysis showed that gender, occupational status, the number of seizures in the past month, and APGAR score were influencing fac-tors of anticipatory grief in patients with epilepsy (P < 0.05). Hierarchical linear model analysis showed that gender and APGAR scorehad significant predictive value for anticipatory grief in patients with epilepsy (P < 0.05). In cross-level interactions, the interaction be-tween APGAR score and gender had a significant impact on anticipatory grief in patients with epilepsy (P < 0.001).Conclusions An-ticipatory grief in patients with epilepsy is at a moderate level and is negatively correlated with family care and social support. Gender,occupational status, the number of seizures in the past month, and family care are influencing factors. Among these, gender and the de-gree of family care have predictive value for anticipatory grief in patients with epilepsy. The degree of family care alleviates (moderates)the positive relationship between gender and anticipatory grief. Therefore, clinical medical staff can implement effective intervention measures to improve the anticipatory grief response by enhancing family care.
