| 目的分析胰腺导管腺癌( PDAC)病人血清外泌体微 RNA-191(miR-191)水平变化,并探讨其诊断和预后价值。方法选取 2017年 6月至 2020年 12月成都市第三人民医院收治的 PDAC病人 68例( PDAC组)、慢性胰腺炎( CP)病人 33例( CP组)和医院体检健康者 41例(健康对照组)为研究对象。采用实时荧光定量 PCR法检测血清外泌体 miR-191的表达水平;分析 PDAC组病人血清外泌体 miR-191水平变化;采用受试者操作特征曲线( ROC曲线)评估血清外泌体 miR-191在 PDAC中的诊断价值,并探讨其与 PDAC组病人临床病理特征的关系;采用 Kaplan-Meier生存曲线法和 Log-Rank检验分析血清外泌体 miR-191水平与 PDAC病人预后的关系。结果 PDAC组病人血清外泌体 miR-191表达水平 2.691±0.892显著高于健康对照组 1.001±0.487及 CP组 1.253±0.656(F=30.09,P<0.001)。血清外泌体 miR-191单独诊断 PDAC的 ROC曲线下面积为 0.82,其诊断效能显著优于糖类抗原 19-9(Z=2.50,P=0.013)。PDAC病人血清外泌体 miR-191高表达与临床 TNM分期( χ2=3.84,P=0.049)和淋巴结转移(χ2=4.77,P=0.029)呈显著相关。此外,血清外泌体 miR-191表达水平较高的 PDAC病人总体生存率较低,差异有统计学意义(Log-Rank χ2=11.63,P<0.001)。结论 PDAC病人血清外泌体 miR-191表达水平明显升高,可能有助于 PDAC的临床诊断及预后评估。 |

| Objective To investigate the changes of serum exosomal MicroRNA 191 (miR-191), as well as its diagnostic and prognos-tic value, in the pancreatic ductal adenocarcinoma (PDAC) patients.Methods A total of 68 PDAC patients and 33 chronic pancreati-tis (CP) patients, who were admitted to The Third People's Hospital of Chengdu and 41 healthy controls undergoing physical examina-tion in the same hospital from June 2017 to December 2020 were enrolled in this study. The levels of serum exosomal miR-191 in these subjects were detected by real-time fluorescence quantitative PCR (RT-qPCR). Changes in serum exosomal miR-191 expression levelswere analyzed in PDAC group. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic efficacy of serumexosomal miR-191 in PDAC patients, and the relationship between serum exosomal miR-191 levels and clinicopathological characteris-tics was also explored. Kaplan-Meier survival curves and Log-Rank test were used to analyze the relationship between serum exosomal miR-191 levels and prognosis of PDAC patients.Results The expression level of serum exosomal miR-191 in PDAC group was signifi-cantly higher than that in control and CP groups (2.691±0.892 vs. 1.001±0.487, 1.253±0.656)(F=30.09,P<0.001). The area under the ROC curve of serum exosomal miR-191 for the diagnosis of PDAC was 0.82. The diagnostic efficacy of serum exosomal miR-191 was better than carbohydrate antigen 19-9(Z=2.50,P=0.013). High expression of serum exosomal miR-191 in PDAC patients showed signif-icant correlation with clinical TNM staging (χ2=3.84,P=0.049) and lymph node metastasis (χ2=4.77,P=0.029).Additionally, PDAC pa-tients with high level of serum exosomal miR-191 showed significantly lower overall survival rate with statistical difference (Log-Rank χ2=11.63, P<0.001).Conclusion Increased expression level of serum exosomal miR-191 in PDAC patients may contribute to the clini-cal diagnosis and prognosis assessment of PDAC. |
