      目的探讨人端粒酶逆转录酶( hTERT)、人吻素( Kisspeptin)在子痫前期病人血清中的表达水平以及与子痫前期的关系。方法选取 2018年 1月至 2022年 12月在陕西省人民医院就诊的 300例子痫前期病人作为观察组,根据病情严重程度将其中 184例轻度子痫前期病人作为轻度子痫前期组,剩余 116例重度子痫前期病人作为重度子痫前期组,同时选取在陕西省人民医院孕检的 150例健康孕妇,作为对照组。比较两组一般资料;观察组和对照组血清 hTERT、Kisspeptin表达水平采用酶联免疫吸附分析( ELISA)进行检测;检测三组血清中总蛋白、清蛋白等生化指标; Pearson法分析子痫前期病人血清 hTERT、 Kisspeptin水平与生化指标之间的相关性; logistic回归分析子痫前期病人血清 hTERT、Kisspeptin水平与子痫前期的关系。结与对照组相比,观察组病人收缩压[( 149.24±20.75)mmHg比( 119.15±15.61)mmHg]和舒张压[( 99.58±14.11)mmHg比(7果3.19±12.80)mmHg]显著高于对照组( P<0.05)。观察组血清中 hTERT[(21.63±3.52)μg/L比( 24.14±0.15)μg/L]、 Kisspeptin水平[(31.54±9.37)ng/L比( 34.61±9.75)ng/L]和清蛋白水平[(32.26±6.18)g/L比( 34.29±5.25)g/L]均显著低于对照组( P<0.05),24 h尿蛋白定量、总蛋白、肌酐水平均显著高于对照组( P<0.05)。重度子痫前期组病人血清中 24 h尿蛋白定量、肌酐水平显著高于轻度子痫前期组( P<0.05)血清中 hTERT、Kisspeptin、清蛋白水平显著低于轻度子痫前期组( P<0.05)。子痫前期病人血清 hTERT、Kisspeptin表达水平与2,4 h尿蛋白定量、总蛋白、肌酐呈负相关,与清蛋白呈正相关( P<0.05)。血清 hTERT、Kisspeptin水平为孕妇患子痫前期的独立保护因素( P<0.05)24 h尿蛋白定量、肌酐为孕妇患子痫前期的独立危险因素( P<0.05)。结论 hTERT、Kisspeptin在子痫前期病人血清中低表达,,二者与 24 h尿蛋白定量、总蛋白、清蛋白、肌酐具有相关性,且均是患子痫前期的独立影响因素。检测血清 hTERT和 Kisspeptin表达水平有望为子痫前期病人的早期筛查和预测提供参考。

      Objective To investigate the expression levels of human telomerase reverse transcriptase (hTERT) and human Kisspeptinin the serum of patients with preeclampsia and their relationship with preeclampsia.Methods A total of 300 cases of preeclampsia pa-tients treated in Shaanxi Provincial People's Hospital from January 2018 to December 2022 were randomly selected as the study group.According to the severity of the disease, 184 cases of mild preeclampsia patients were selected as the mild preeclampsia group, and theremaining 116 cases of severe preeclampsia patients were selected as the severe preeclampsia group. Meanwhile, 150 healthy pregnantwomen who underwent pregnancy examination in our hospital were randomly selected as a control group. The general information of thetwo groups was compared; enzyme linked immunosorbent assay (ELISA) was applied to detect the expression levels of hTERT and Kiss-peptin in serum of the study group and the control group; biochemical indicators such as total protein and albumin in serum of threegroups were detected; Pearson method was applied to analyze the correlation between serum hTERT, Kisspeptin levels and biochemicalindicators in patients with preeclampsia; logistic regression was applied to analyze the relationship between serum hTERT,Kisspeptinlevels and preeclampsia in patients with preeclampsia.Results Compared with the control group, the systolic blood pressure [(149.24± 20.75)mmHg vs. (119.15±15.61)mmHg] and diastolic blood pressure [(99.58±14.11)mmHg vs. (73.19±12.80)mmHg] in the study group were obviously higher than those in the control group (P<0.05). The levels of hTERT [(21.63±3.52)μg/L vs. (24.14±0.15)μg/L], Kiss-peptin[(31.54±9.37)ng/L vs. (34.61±9.75)ng/L], and albumin [(32.26±6.18)g/L vs. (34.29±5.25)g/L] in the serum in the study group were obviously lower than those in the control group (P<0.05), while the levels of 24-hour urine protein quantification, total protein, and cre-atinine were obviously higher than those in the control group (P<0.05). The levels of 24-hour urine protein quantification and creatininein the severe preeclampsia group were obviously higher than those in the mild preeclampsia group (P<0.05), while the levels of hTERT, Kisspeptin, and albumin in the serum were obviously lower than those in the mild preeclampsia group (P<0.05). The expression levels of hTERT and Kisspeptin in serum of patients with preeclampsia were negatively correlated with 24-hour urine protein quantification, total protein, and creatinine, while positively correlated with albumin (P<0.05). Serum hTERT and Kisspeptin levels were independent protective factors for preeclampsia in pregnant women (P<0.05), while 24-hour urine protein quantification and creatinine were inde-pendent risk factors for preeclampsia in pregnant women (P<0.05).Conclusions The expressions of hTERT and Kisspeptin are low in the serum of patients with preeclampsia, and they are related to 24-hour urine protein quantification, total protein, albumin and creati-nine, and are independent influencing factors of preeclampsia. The detection of serum hTERT and Kisspeptin expression levels is ex-pected to provide reference for early screening and prediction of preeclampsia patients.
