| 目的探索体外膜肺氧合( ECMO)支持下行全脑血管造影及动脉瘤栓塞的诊疗经验。方法总结河南省直第三人民医院放射介入科 2023年 2月 15日收治的 1例颅内动脉瘤破裂致蛛网膜下腔出血在 ECMO支持下进行动脉瘤栓塞病人急诊抢救过程。结果病人主诉头晕呕吐 4h，意识障碍 2h。术前诊断:脑出血。入院后 2h出现严重低氧血症，经传统机械通气无效，予以早期实施动脉 -静脉 ECMO，后行全脑血管造影及 Neuroform Atlas支架辅助弹簧圈栓塞动脉瘤手术，成功救治病人。结论置入 ECMO的动脉瘤破裂病人可在恢复血流动力学相关指征后行支架辅助下动脉瘤栓塞术进行去除病因治疗。 |

| Objective To explore the diagnosis and treatment experience of total cerebral angiography and aneurysm embolizationsupported by extracorporeal membrane oxygenation (ECMO).Methods The emergency rescue process of a patient with subarachnoidhemorrhage caused by ruptured intracranial aneurysm treated in the Department of radiation intervention of He'nan No.3 ProvincialPeople's Hospital on Feb. 15, 2023, with the support of ECMO was summarized. Results The patient complained of dizziness andvomiting for 4 hours and disturbance of consciousness for 2 hours. Preoperative diagnosis was cerebral hemorrhage. Severe hypoxemiaoccurred 2 hours after admission, and traditional mechanical ventilation was ineffective. Arterial-venous ECMO was performed early, followed by total cerebral angiography and Neuroform Atlas stent-assisted coil embolization of aneurysms, and the patients were suc-cessfully treated.Conclusion Patients with ruptured aneurysms implanted with ECMO can be treated by stent-assisted aneurysm em-bolization after the restoration of hemodynamic indications. |
