      目的评估不同保存条件下脂溶性维生素全血、血浆及液液萃取后(前处理后)样本的稳定性。方法选取 2023年 8— 10月于北京航天总医院就诊的 100例病人作为研究对象,根据保存条件分为五组:全血室温组、血浆室温组、血浆冷藏组、血浆冷冻组、前处理后样本组,每组 20例。采用高效液相色谱串联质谱法( high performance liquid chromatography-tandem mass spec-trometry,HPLC-MS/MS)测定不同保存条件下样本中维生素 A(vitamin A,VA)、 25羟维生素 D[25-hydroxyvitamin D,25(OH)D]、 α-维生素 E(α-vitamin E,α-VE)的含量,与即刻值比较,差异率超过总变化限( total change limit,TCL)被认为不可接受。结果 VA前处理后样本的即刻检测值为( 421.16±122.61)μg/L,自动进样器中放置 7d后的检测值为( 348.52±120.49)μg/L,差异率超过 TCL,其他化合物的不同保存条件下差异率均在 TCL范围内。 VA、25(OH)D和 α-VE全血样本室温放置 12 h后测量值有下降趋势, VA、25(OH)D、α-VE全血室温放置 7d后测量值与即刻值相比,差异无统计学意义( P=0.226、0.552、0.617)。 VA血浆样本室温、冷藏和冷冻 24 h后测量值有下降趋势, 25(OH)D和 α-VE血浆样本室温和冷藏 24 h、冷冻 7d后测量值有下降趋势,不同保存时间点检测结果与即刻值相比,差异无统计学意义( P>0.05)。结论 VA、25(OH)D和 α-VE全血样本室温保存可稳定 7d,血浆样本室温和冷藏可稳定 7d,冷冻可稳定 1个月,前处理后样本自动进样器内可稳定 24 h。分离血浆后冷冻保存可延长样本检测稳定性,实验室可根据实际检测情况对脂溶性维生素样本进行保存。

      Objective To evaluate the stability of whole blood, plasma and liquid-liquid extraction samples of fat-soluble vitamins un-der different storage conditions.Methods A total of 100 patients treated in Beijing Aerospace General Hospital from August to Octo-ber 2023 were selected as the study objects and divided into 5 groups according to storage conditions: whole blood room temperaturegroup, plasma room temperature group, plasma refrigeration group, plasma freezing group and treated sample group, with 20 samples ineach group. High performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) was used to determine vitamin A (VA), 25 hydroxyvitamin D [25(OH)D] and α-vitamin E (α-VE) in samples under different storage conditions. When compared to imme-diate values, a difference rate exceeding the total change limit (TCL) was considered unacceptable.Results The immediate detection value of the sample after VA pretreatment was (421.16±122.61) μg/L and the detection value after being placed in the autosampler for7 days was (348.52±120.49) ng/L. The difference rate exceeded that of TCL. The difference rates of other compounds under differentstorage conditions were all within the range of TCL. The measured values of VA, 25(OH)D and α-VE blood samples at room tempera-ture decreased for 12 h while the measured values of VA, 25(OH)D and α-VE blood samples at room temperature for 7 days showed no statistical significance compared with the immediate values (P = 0.226, 0.552, 0.617). VA plasma samples showed a downward trend atroom temperature, refrigerated, frozen for 24 h and 25(OH)D, α-VE plasma samples showed a downward trend at room temperature, re-frigerated for 24 h and frozen for 7 days. There was no statistical significance between the detection results at different storage timepoints and the immediate value (P > 0.05).Conclusions Whole blood samples of VA, 25(OH)D and α-VE can be stable for 7 days atroom temperature. Plasma samples can be stable for 7 days at room temperature, cold storage and frozen for 1 month. After treatment,samples can be stable for 24 hours in the automatic sampler. Frozen storage after plasma separation can prolong the stability of sampledetection and the laboratory can store fat-soluble vitamin samples according to the actual detection situation.
