      目的探讨肝细胞癌(HCC)病人血清微 RNA(miR)-452-5p的表达及其联合甲胎蛋白( AFP)异常凝血酶原( PIVKA-Ⅱ)对 HCC的诊断价值。方法选取 2020年 1月至 2023年 5月保定市第二中心医院收治的 HCC病人11、3例( HCC组)、肝硬化病人 65例(肝硬化组)和体检正常者 60例(健康组)。采用实时定量 PCR法检测 miR-452-5p表达水平,化学发光法检测血清 AFP及 PIVKA-Ⅱ水平,并比较 HCC组、肝硬化组和健康组血清 miR-452-5p、AFP及 PIVKA-Ⅱ水平,分析血清 miR-452-5p表达水平与 HCC病人临床特征的关系。应用受试者操作特征曲线( ROC曲线)分析血清 miR-452-5p、AFP及 PIVKA-Ⅱ水平对 HCC的诊断价值。采用 Pearson相关性分析检验 HCC病人血清 miR-452-5p表达水平与 AFP及 PIVKA-Ⅱ的相关性。结果 HCC组血清 miR-452-5p(4.48±1.33比 1.05±0.36,0.34±0.05)、 AFP[( 492.15±68.24)μg/L比( 21.46±9.13)μg/L,(3.52±0.40)μg/L]及 PIVKA-Ⅱ[(326.72±68.35)AU/L比( 21.75±6.92)AU/L,(18.50±4.36)AU/L]水平均明显高于肝硬化组和健康组( P<0.001)。 Ⅲ~Ⅳ期、低分化、肿瘤数目多发、门静脉癌栓、远处转移、 AFP及 PIVKA-Ⅱ阳性的 HCC病人血清 miR-452-5p表达水平明显升高( P<0.001)。 ROC曲线显示, miR-452-5p、AFP及PIVKA-Ⅱ三项联合诊断 HCC的AUC[0.98,95%CI:(0.96,1.00)]最大,其灵敏度(98.23%)最高。相关性分析显示, HCC病人血清 miR-452-5p水平与 AFP和 PIVKA-Ⅱ均呈正相关(r=0.80、0.84,均 P<0.001)。结论 miR-452-5p在 HCC病人中呈高表达,其联合 AFP及 PIVKA-Ⅱ检测对 HCC的诊断价值更大。

      Objective To investigate the expression of miR-452-5P in patients with hepatic cellular cancer(HCC) and the value of its combination with AFP and abnormal prothrombin (PIVKA-Ⅱ) in the diagnosis of HCC.Methods 113 HCC patients (HCC group), 65cirrhosis patients (cirrhosis group), and 60 normal individuals (healthy group) admitted to Baoding Second Central Hospital from Janu-ary 2020 to May 2023 were selected. The expression level of miR-452-5p was detected by real-time quantitative PCR, and the levels of serum AFP and PIVKA-Ⅱ were detected by chemiluminescence. The levels of serum miR-452-5p, AFP and PIVKA-Ⅱ in HCC group,liver cirrhosis group and control group were compared. To analyze the relationship between the expression level of serum miR-452-5pand the clinical characteristics of HCC patients. Receiver operating characteristic curve (ROC curve) curve was used to analyze the di-agnostic value of serum miR-452-5p, AFP and PIVKA-Ⅱ levels for HCC. The correlation between miR-452-5p expression level and AFP and PIVKA-Ⅱ in HCC patients was analyzed by Pearson correlation analysis.Results The levels of serum miR-452-5p (4.48±1.33 vs. 1.05±0.36, 0.34±0.05), AFP [(92.15±68.24) μg/L vs. (21.46±9.13) μg/L,(3.52±0.40) μg/L] and PIVKA-Ⅱ [(326.72±68.35) AU/ L vs. (21.75±6.92) AU/L, (18.50±4.36) AU/L] in HCC group were significantly higher than those in liver cirrhosis group and healthygroup (P<0.001). The serum miR-452-5p expression levels are significantly elevated in HCC patients with stage Ⅲ-Ⅳ, low differentia-tion, multiple tumors, portal vein cancer thrombi, distant metastasis, AFP and PIVKA-Ⅱ positivity (P<0.001). ROC curve showed that miR-452-5p, AFP and PIVKA-Ⅱ combined diagnosis of HCC had the highest AUC [0.98, 95%CI: (0.96,1.00)], and their sensitivity (98.23%) was the highest. Correlation analysis showed that the level of serum miR-452-5p in HCC patients were positively correlated with AFP and PIVKA-Ⅱ (r=0.80, 0.84, both P<0.001).Conclusion MiR-452-5p is highly expressed in HCC patients, and its combi-nation with AFP and PIVKA-Ⅱ detection has high value in the diagnosis of HCC.
