| 目的探讨血清微 RNA-140(miR-140)和微 RNA-146a(miR-146a)联合检测在肺结核合并肺部细菌感染早期诊断中的临床价值。方法根据整群抽样选取 2021年 10月至 2022年 10月武汉市金银潭医院收治的肺结核合并肺部细菌感染病人 112例为研究组,同期采用随机数字表法选取该院收治单纯肺结核 150例,排除不同意观察的 38例,最后纳入 112例作为对照组。采用实时荧光定量 PCR(RT-qPCR)检测 miR-140和 miR-146a的表达水平;受试者操作特征曲线( ROC曲线)分析 miR-140和 miR-146a对肺结核合并肺部细菌感染的诊断价值。结果与对照组相比,研究组内毒素、红细胞沉降率(ESR)、 C反应蛋白(CRP)、白细胞计数水平显著升高(均 P<0.05)。与对照组相比,研究组血清 miR-140的表达水平显著升高( 1.32±0.25比 1.01±0.12),miR-146a的表达水平显著降低( 0.86±0.15比 1.03±0.11)(均 P<0.05)。肺结核合并肺部细菌感染病人血清中 miR-140与 miR-146a表达水平呈负相关( r=.0.36,P<0.001)。二者联合诊断肺结核合并肺部细菌感染的曲线下面积( AUC)高于 miR-140、 miR-146a单独诊断的 AUC值(均 P<0.001)。结论肺结核合并肺部细菌感染病人血清 miR-140的表达水平升高, miR-146a的表达水平降低,联合检测能够提高诊断效能。 |

| Objective To explore the clinical value of combined detection of serum microRNA-140 (miR-140) and microRNA-146a (miR-146a) in the early diagnosis of pulmonary tuberculosis complicated with pulmonary bacterial infection.Methods Using clustersampling, 112 patients with pulmonary tuberculosis combined with pulmonary bacterial infection admitted to Wuhan Jinyintan Hospitalfrom October 2021 to October 2022 were selected as the study group. During the same period, 150 patients with simple pulmonary tu-berculosis were selected using a random number table method. After excluding 38 patients who declined participation, 112 patientswere finally included as the control group. Real-time quantitative PCR (RT-qPCR) was used to detect the expression levels of miR-140 and miR-146a. Receiver operating characteristic curve (ROC curve) analysis was performed to evaluate the diagnostic value of miR-140 and miR-146a for pulmonary tuberculosis complicated with pulmonary bacterial infection.Results Compared with the control group, levels of endotoxin, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), and white blood cell count were significantly higher in the study group (all P<0.05). The expression level of serum miR-140 was significantly higher in the study group (1.32±0.25 vs. 1.01± 0.12), while miR-146a expression was significantly lower (0.86±0.15 vs. 1.03±0.11) (both P<0.05). Correlation analysis revealed a nega-tive correlation between serum miR-140 and miR-146a expression levels in patients with pulmonary tuberculosis and pulmonary bacte-rial infection (r=.0.36, P<0.001). The area under the curve (AUC) for the combined detection of miR-140 and miR-146a in diagnosingpulmonary tuberculosis complicated with pulmonary bacterial infection was higher than that for either miRNA alone (both P<0.001). Conclusions Patients with pulmonary tuberculosis complicated with pulmonary bacterial infection exhibit significantly increased se-rum miR-140 expression and decreased miR-146a expression. Combined detection of these miRNAs improves diagnostic performance. |
