| 目的探讨瘦素( Leptin)、血管黏附蛋白 -1(VAP-1)在 2型糖尿病( T2DM)下肢血管病变( LVD)病人中的表达及与胰岛素抵抗( HOMA-IR)、炎症反应的相关性。方法选取 2019年 5月至 2021年 10月邯郸市中心医院收治的 180例 T2DM病人为研究对象,根据踝肱指数( ABI)将病人分两组:合并 LVD组( n=85)未合并 LVD组( n=95);基于年龄、性别,按照随机数字表法选择 80例健康体检志愿者为健康组。收集一般资料,采用酶联免疫吸,附分析检测血清中 Leptin、VAP-1表达水平, Pearson法分析 Leptin、VAP-1与炎症反应指标表达的相关性,采用 logistic回归分析影响 LVD的因素。结果与未合并 LVD组和健康组相比,合并 LVD组病人血清 Leptin[( 7.30±0.48)ng/L比( 5.66±0.86)ng/L、(4.89±0.91)ng/L]、 VAP-1[( 149.83±30.51)μg/L比(106.24±15.28)μg/L、(63.92±19.22)μg/L]、甘油三酯、糖化血红蛋白( HbA1c)、空腹胰岛素( FINS)、胰岛素抵抗指数( HOMA-IR)、超敏 C反应蛋白( hs-CRP)、肿瘤坏死因子 α(TNF-α)、白细胞介素 -6(IL-6)水平均上升,白蛋白、肾小球滤过率( eGFR)降低,差异有统计学意义( P<0.05);合并 LVD病人血清中 Leptin与 VAP-1表达呈正相关( r=0.42,P<0.05);合并 LVD病人血清 Leptin、VAP-1水平与甘油三酯、 HbA1c、FINS、HOMA-IR、hs-CRP、TNF-α、IL-6水平均呈正相关( P<0.05),与白蛋白表达呈负相关( P<0.05); logistic回归结果显示 HOMA-IR、Leptin、VAP-1均可能是影响 T2DM合并 LVD的因素( P<0.05)。结论 Leptin、 VAP-1在 T2DM合并 LVD病人血清中高表达,二者表达与病人胰岛素抵抗及炎症反应密切相关。 |

| Objective To investigate the expression of Leptin and vascular adhesion protein-1 (VAP-1) in patients with type 2 diabe-tes mellitus (T2DM) lower extremity vascular disease (LVD) and their correlation with insulin resistance (HOMA-IR) and inflammatory response.Methods A total of 180 patients with T2DM who were admitted to Handan Central Hospital from May 2019 to October 2021were regarded as the research objects, and the patients were grouped into two groups according to the ankle brachial index (ABI): thecomplicated LVD group (n=85) and the uncomplicated LVD group (n= 95); according to age and gender, 80 healthy volunteers were re-garded as the health group using a random number table method. The general information was collected, the expression levels of Leptinand VAP-1 in serum were detected by enzyme-linked immunosorbent assay. Pearson method was used to analyze the correlation be-tween Leptin, VAP-1 and the expression of inflammatory response indicators, and logistic regression was used to analyze factors affect-ing LVD.Results Compared with the uncomplicated LVD group and the health group, the serum levels of Leptin [(7.30±0.48) ng/L vs. (5.66±0.86) ng/L, (4.89±0.91)ng/L], VAP-1 [(149.83±30.51) μg/L vs. (106.24±15.28) μg/L, (63.92±19.22) μg/L], triglycerides, glycat-ed hemoglobin (HbA1c), fasting insulin (FINS), insulin resistance index (HOMA-IR), high-sensitivity C-reactive protein (hs-CRP), tu-mor necrosis factor α (TNF-α), interleukin 6 (IL-6) in the complicated LVD group were all increased (P<0.05), and albumin, Glomeru-lar filtration rate (eGFR) decreased; the difference was statistically significant (P<0.05); the expressions of Leptin and VAP-1 in serum of patients with LVD were positively correlated (r=0.42, P<0.05). The levels of serum Leptin and VAP-1 in patients with LVD were pos-itively correlated with the levels of triglycerides, HbA1c, FINS, HOMA-IR, hs-CRP, TNF-α and IL-6(P<0.05), and it was negatively correlated with albumin expression (P<0.05); Logistic regression results showed that HOMA-IR, Leptin, and VAP-1 were all factors af-fecting T2DM complicated with LVD (P<0.05). Conclusion Leptin and VAP-1 are highly expressed in the serum of patients withT2DM LVD, and their expressions are closely related to insulin resistance and inflammatory response in patients. |
