| 目的将外用生长因子在头面部深 Ⅱ度烧伤创面中的最佳治疗证据应用到临床实践并评价其效果。方法该研究于 2020年 6月至 2021年 9月在联勤保障部队第九一〇医院开展,纳入 72例头面部深 Ⅱ度烧伤病人,按照随机数字表法分为试验组( 36例)和对照组( 36例)。试验组采用循证策略下的外用生长因子治疗方案,对照组采用常规碘伏换药。比较两组病人的创面愈合时间、溶痂时间、住院时间、疼痛评分、瘢痕评分及不良反应发生情况。结果循证证据共纳入 1篇专家共识、 2篇临床决策、 2篇相关指南及 7篇临床研究。临床实践结果显示,试验组愈合时间为(15.19±1.60)d,显著短于对照组的(17.63±2.03) d(P<0.05)。疼痛评分方面,治疗后 14 d试验组为( 0.26±0.09)分,对照组为( 0.93±0.81)分,差异有统计学意义( P<0.05)。瘢痕评分方面,治疗 12个月时试验组为( 2.01±0.61)分,对照组为( 3.63±0.61)分,差异有统计学意义( P<0.05)。结论基于证据的循证实践能够有效缩短头面部深 Ⅱ度烧伤病人创面愈合时间、降低其疼痛以及瘢痕评分,保证了外用生长因子治疗技术的医疗护理质量。 |

| Objective To translate the optimal therapeutic evidence of topical growth factors in deep second-degree burn wounds on the head and face into clinical practice and to evaluate their effectiveness.Methods From June 2020 to September 2021, this studywas conducted in 910 Hospital of The Logistic Support Force. A total of 72 patients with deep second-degree burns of the head and face were enrolled and randomly assigned to the experimental group (n=36) and the control group (n=36). The experimental group received an evidence-based topical growth factor protocol, while the control group received conventional iodine disinfection and dressing chang-es. Wound healing time, scab dissolution time, length of hospital stay, pain scores, scar scores, and adverse reactions were compared be-tween the two groups.Results A total of one expert consensus, two clinical decisions, two related guidelines, and seven clinical re-search articles were included as evidence. Clinical practice showed that the healing time in the experimental group was (15.19±1.60) d,significantly shorter than (17.63±2.03) d in the control group (P<0.05). Regarding pain scores, it was (0.26±0.09) points in the experi-mental group compared with (0.93±0.81) points in the control group at day 14. There was a statistically significant difference betweenthe two groups (P<0.05). For scar scores, at 12 months, the experimental group scored (2.01±0.61) points, which was significantly lowerthan the score of (3.63±0.61) in the control group (P<0.05).Conclusion Evidence-based practice can effectively shorten healing time and decrease pain and scar scores in patients with deep second-degree head and facial burns, thus ensuring the quality of medical care with topical growth factor therapy techniques. |
