| 目的探讨大便失禁管理器联合负压治疗会阴肛周大范围皮肤和软组织念珠菌属感染经验。方法总结 2021年 7月 9日广州市红十字会医院收治 1例 56岁病人会阴肛周大范围皮肤和软组织念珠菌属感染的治疗方案。结果经行会阴肛周创面清创、大便失禁管理器套件植入及封闭式负压引流术( VAC)，创面感染得到控制，再次行会阴肛周创面清创植皮、头部取皮术、封闭式负压引流术封闭创面；入院第 32天，创面愈合，予以出院。结论会阴肛周大范围皮肤和软组织念珠菌属感染治疗要点为入院后尽早行创面清创、抗感染治疗、维持生命体征平稳；由于部位特殊、创面易受大便污染、封闭式负压材料难以固定等难题，大便失禁管理器联合 VAC治疗是一种有效的解决办法。 |

| Objective To explore the experience of fecal incontinence manager combined with negative pressure in the treatment of large-scale perineal skin and soft tissue infection.Methods The treatment protocol for a 56-year-old patient with extensive perianaland perineal skin and soft tissue Candida infection who was admitted to Guangzhou Red Cross Hospital on July 9, 2021 was summa-rized.Results After perineal wound debridement, fecal incontinence manager kit implantation and closed negative pressure drainage(VAC), wound infection was controlled. Then perineal wound debridement and skin grafting, head skin removal, closed negative pres-sure drainage were performed to close the wound. On the 32nd day of admission, the wound healed and he was discharged.Conclu. sions The main points of diagnosis and treatment of large-scale perineal skin and soft tissue infection are wound debridement as soon as possible after admission, anti-infection treatment and maintaining stable vital signs. Fecal incontinence manager combined withVAC treatment is an effective solution because of the special site, wound easily contaminated by stool, and closed negative pressure ma-terial difficult to fix. |
