      目的探究合并人巨细胞病毒( HCMV)感染冠心病病人外周血辅助性 T细胞 17/调节性 T细胞( Th17/Treg)相关细胞因子表达、可溶性白细胞分化抗原 14(sCD14)水平变化及与预后的关联。方法回顾性分析 2020年 5月至 2022年 1月徐州市中心医院 139例冠心病合并 HCMV感染病人(感染组)及 111例冠心病未合并 HCMV感染病人(未感染组)的临床资料,比较感染组及未感染组的外周血 Th17/Treg相关细胞因子表达、 sCD14水平,采用 Pearson检验分析各指标与 HCMV感染 -DNA载量的相关性;根据随访 1年期间病人不良心血管事件及主要终点事件发生情况分为预后较好组及预后不良组,采用 logistic回归分析 HCMV感染及外周血 Th17/Treg相关细胞因子表达、 sCD14水平变化对冠心病病人预后的影响。结果感染组的 Th17细胞比例、 Th17/Treg、白细胞介素 -17(IL-17)、 sCD14水平高于未感染组[( 16.81±3.45)%、5.20±1.23、(18.27±3.63)ng/L、

      Objective To explore the changes of expressions of peripheral blood T helper cell 17/T regulatory cell (Th17/Treg) cyto-kines and level of soluble cluster of differentiation antigen 14 (sCD14) in patients with coronary heart disease complicated with humancytomegalovirus (HCMV) infection and their association with prognosis.Methods The clinical data of 139 patients with coronary heart disease and HCMV infection (infected group) and 111 patients with coronary heart disease but without HCMV infection (non-infected group) admitted to Xuzhou Central Hospital between May 2020 and January 2022 were retrospectively analyzed. The expressions of pe-ripheral blood Th17/Treg cytokines and level of sCD14 were compared between infected group and non-infected group, and the correla-tion between each indicator and HCMV infection-DNA load was analyzed by Pearson test. According to the occurrence of adverse car-diovascular events and primary endpoint events during 1-year follow-up, patients were assigned into good prognosis group and poorprognosis group. Logistic regression analysis was used to analyze the influence of HCMV infection and changes of peripheral bloodTh17/Treg cytokines and sCD14 on the prognosis of patients with coronary heart disease.Results The proportion of Th17 cell, Th17/ Treg and levels of interleukin-17 (IL-17) and sCD14 in infected group were higher than those in non-infected group [(16.81±3.45)%, 5.20±1.23, (18.27±3.63) ng/L, (47.06±6.45) μg/L vs. (13.26±2.53)% , 2.61±0.46, (15.49±3.12) ng/L, (33.91±6.17) μg/L] (P<0.001), while the proportion of Treg cell and level of transforming growth factor-β (TGF-β) were lower than those in non-infected group [(3.23± 0.64)%, (9.31±1.58) ng/L vs. (5.09±1.32)%, (13.53±2.28) ng/L] (P<0.001). Th17/Treg and sCD14 level in patients with coronary heartdisease and HCMV infection were positively correlated with HCMV-DNA load copy logarithm value (r=0.29, 0.27, P=0.008, 0.013). The HCMV infection rate, Th17/Treg, IL-17 and sCD14 in poor prognosis group were higher than those in good prognosis group while the TGF-β level was lower than that in good prognosis group (P<0.05). Binary logistic regression analysis was conducted by taking HC-MV infection, expressions of Th17/Treg cytokines and sCD14 level as independent variables and taking prognosis of patients with coro-nary heart disease as a dependent variable, and it was found that HCMV infection and increases in Th17/Treg, IL-17 and sCD14 were risk factors affecting the prognosis of patients with coronary heart disease, and the decrease of TGF-β level was a protective factor (P< 0.05).Conclusions Abnormal changes in the expressions of peripheral blood Th17/Treg cytokines and sCD14 level in patients withcoronary heart disease and HCMV infection are related to HCMV-DNA load. The Th17/Treg cytokines and sCD14 in peripheral bloodof patients may serve as prognostic markers in patients with coronary heart disease complicated with HCMV infection.
