      目的探讨关节镜辅助下微骨折技术( MF)联合富血小板血浆( PRP)注射治疗膝骨关节炎( KOA)的效果。方法 选取 2021年 1月至 2022年 12月秦皇岛市第二医院收治的 80例 KOA病人,并按随机数字表法分成 A组[n=40,实施关节镜下 MF联合玻璃酸钠(HA)注射治疗]和 B组( n=40,实施关节镜下 MF联合 PRP注射治疗)。比较两组疗效、关节液中炎症因子[白细胞介素 -1β(IL-1β)肿瘤坏死因子 α(TNF-α)基质金属蛋白酶 3(MMP-3)]、氧化应激水平[超氧化物歧化酶( SOD)、一氧化氮(NO)]、患膝功能[采,用 Lysholm膝关节功能评分,表评定]及并发症情况。结果 B组总有效率为 95.00%,比 A组的 80.00%高( P<0.05);术后 3个月,两组关节液中 IL-1β、TNF-α、MMP-3及 NO均低于术前( P<0.05),且 B组均低于 A组( P<0.05);术后 3个月,两组关节液 SOD及膝关节评分均高于术前( P<0.05)且 B组均高于 A组( P<0.05); B组并发症总发生率 5.00%和 A组的 12.50%比较差异无统计学意义( P>0.05)。结论对 KOA实施关节镜下 MF联合 PRP注射治疗的效果显著,可有效调节病病人,人关节滑液炎症因子、氧化应激水平,明显改善患膝功能。

      Objective To investigate the effect of arthroscopic assisted microfracture technique (MF) combined with platelet richplasma (PRP) injection in treating knee osteoarthritis (KOA) .Methods Eighty KOA patients, admitted to The Second Hospital of Qin-huangdao from January 2021 to December 2022, were selected and randomly assigned to group A [n=40, treated with arthroscopic MF combined with sodium hyaluronate (HA) injection] or group B (n=40, treated with arthroscopic MF combined with PRP injection). The efficacy, inflammatory factors in joint fluid [interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), matrix metalloproteinase-3 (MMP-3)], oxidative stress levels [superoxide dismutase (SOD), nitric oxide (NO)], knee function [evaluated using the Lysholm knee scoringscale], and complications were compared between the two groups.Results The total effective rate of group B was 95.00%, which was higher than 80.00% in group A (P<0.05). Three months after surgery, the levels of IL-1β, TNF-α, MMP-3, and NO in the joint fluid of both groups were lower than those before surgery (P<0.05), and the levels in group B were lower than those in group A (P<0.05); three months after surgery, both groups had higher levels of joint fluid SOD and knee joint scores than they were before surgery (P<0.05), and group B had higher levels than group A (P<0.05). There was no difference in the total incidence of complications between group B (5.00%) and group A (12.50%) (P>0.05).Conclusion The effect of arthroscopic MF combined with PRP injection treatment on KOApatients is obvious, which can effectively regulate the levels of inflammatory factors and oxidative stress in the joint fluid of patients,and obviously improve knee function.
