      目的探究血清血管内皮生长因子( VEGF)、内皮素 1(ET-1)水平与支气管哮喘( BA)病人病情程度的相关性,并分析其在激素治疗敏感性中的交互作用。方法选取 2021年 5月至 2022年 5月廊坊市第四人民医院 230例 BA病人,根据病情程度分为轻度组( n=82)、中度组( n=77)和重度组( n=71)另选同期同年龄段 70例健康志愿者作为健康组,开展前瞻性研究。比较各组一般资料、血清 VEGF、ET-1、外周血炎症因子[血嗜,酸性粒细胞计数( EOS)、 C反应蛋白( CRP)、降钙素原( PCT)]水平。分析 BA病人血清 VEGF、ET-1水平与病情程度、外周血炎症因子水平的相关性。所有 BA病人均接受糖皮质激素治疗,比较激素敏感、激素抵抗病人治疗前后血清 VEGF、ET-1水平及变化值,交互作用分析治疗前血清指标在激素治疗敏感性中的作用。结重度组、中度组、轻度组血清 VEGF、ET-1、CRP、PCT水平及外周血 EOS计数高于健康组,且不同病情程度病人血清各指标比果较差异有统计学意义( P<0.05);重度组、中度组、轻度组血清 VEGF水平分别为( 82.13±1.56)ng/L、(77.46±1.28)ng/L、(75.39±0.97)ng/L,高于健康组的( 8.06±1.41)ng/L,重度组、中度组、轻度组血清 ET-1水平分别为( 170.32±20.17)ng/L、(158.63±17.92) ng/L、(115.43±12.81)ng/L,高于健康组的( 61.85±9.79)ng/L(P<0.05);血清各指标水平与病情程度、 CRP、PCT水平及外周血 EOS计数呈正相关( P<0.05);激素抵抗者治疗前后血清 VEGF、ET-1水平高于激素敏感者, VEGF、ET-1水平变化值低于激素敏感者(P<0.05); VEGF、ET-1高变化在激素抵抗中呈正向交互作用( P<0.05)。结论 BA病人血清 VEGF、ET-1水平升高与病情程度有关,其水平变化在激素抵抗中呈正向交互作用。

      Objective To investigate the correlation between serum levels of vascular endothelial growth factor (VEGF) and endothe-lin-1 (ET-1) and the severity of bronchial asthma (BA) in patients, and to analyze their interaction in hormone therapy sensitivity.Meth. ods A total of 230 BA patients in the Fourth People's Hospital of Langfang City from May 2021 to May 2022 were selected and divid-ed into mild group (n=82), moderate group (n=77) and severe group (n=71) according to the severity of the disease, another 70 healthyvolunteers of the same age group were selected as the healthy group to carry out a prospective study. The general data, serum VEGF,ET-1, peripheral blood inflammatory factors [eosinophil count (EOS), C-reactive protein (CRP), procalcitonin (PCT)] levels were com-pared among the groups. The correlation between serum VEGF, ET-1 levels and disease severity, peripheral blood inflammatory factorlevels in BA patients was analyzed. All patients with BA were treated with glucocorticoids. The levels and changes of serum VEGF andET-1 before and after treatment in patients with hormone sensitivity and hormone resistance were compared, and the role of serum in-dexes before treatment in the sensitivity of hormone therapy was analyzed by interaction.Results The levels of serum VEGF, ET-1,CRP, PCT and peripheral blood EOS count in the severe group, moderate group and mild group were higher than those in the healthygroup, and the differences of serum indexes in patients with different disease severity were statistically significant (P<0.05). The levelsof serum VEGF in severe group, moderate group and mild group were (82.13±1.56) ng/L, (77.46±1.28) ng/L and (75.39±0.97) ng/L, re-spectively, which were higher than those in healthy group [(8.06±1.41) ng/L], and the levels of serum ET-1 in severe group, moderategroup and mild group were (170.32±20.17) ng/L, (158.63±17.92) ng/L and (115.43±12.81) ng/L, respectively, which were higher thanthose of the healthy group [(61.85±9.79) ng/L] (P<0.05). The levels of serum indexes were positively correlated with the severity of thedisease, CRP, PCT levels and peripheral blood EOS count (P<0.05). The levels of serum VEGF and ET-1 in patients with hormone re-sistance before and after treatment were higher than those in patients with hormone sensitivity, and the changes of VEGF and ET-1 lev-els were lower than those in patients with hormone sensitivity (P<0.05). The high changes of VEGF and ET-1 showed a positive interac-tion in hormone resistance (P<0.05).Conclusion The increase of serum VEGF and ET-1 levels in BA patients is related to the severi-ty of the disease, and the level changes have a positive interaction in hormone resistance.
