| 目的观察急性脑梗死( ACI)病人血清 CD5抗原样蛋白(CD5L)和神经元正五聚蛋白 2(NPTX-2)水平变化,并探讨其表达与病人病情严重程度及对短期预后的预测价值。方法选择 2021年 1月至 2023年 4月在保定市第一中心医院收治的 108例 ACI病人,根据美国国立卫生研究院卒中量表( NIHSS)分为轻度组( n=46)、中度组( n=50)、重度组( n=12);根据改良 Rankin量表( mRS)评分分为预后不良组( n=26)、预后良好组( n=82);收集病人的临床资料;病人血清 CD5L和 NPTX-2水平选择酶联免疫吸附分析( ELISA)检测; Spearman分析 CD5L,NPTX-2水平与 NIHSS、mRS评分相关性; logistic分析影响因素;受试者操作特征曲线( ROC)分析诊断价值。结果重度组 CD5L(147.11±19.85)ng/L、NPTX-2(145.17±18.06)ng/L水平低于轻、中度组( P< 0.05),中度组 CD5L、NPTX-2(201.61±23.59)ng/L、(214.81±28.67)ng/L水平低于轻度组( 215.83±28.64)ng/L、(232.97±30.08)ng/L(P<0.05);预后不良组血清 CD5L、NPTX-2水平低于预后良好组( P<0.001)。 CD5L水平与 NIHSS、mRS评分呈负相关( r=.0.51、 .0.54,P<0.05)NPTX-2水平与 NIHSS、mRS评分呈负相关( r=.0.47、.0.49,P<0.05)。血清 CD5L、NPTX-2是 ACI病人预后不良的影响因素( P<0.05)。血清 CD5L、NPTX-2以及二者联合诊断 ACI病人发生短期不良预后的 ROC曲线下面积( AUC)分别为[0.86,95%CI:(0.78,0.92)]、[0.87,95%CI:(0.80,0.93)]和[0.96,95%CI:(0.90,0.99)]二者联合诊断效能高于单独检测效能(Z=2.41,P=0.016;Z=2.18,P=0.029)。结论 ACI病人血清 CD5L、NPTX-2低表达可其病情严重程度及预后有重要联系,能与,且二者单独及联合诊断对病人发生短期不良预后均具有一定预测价值。 |

| Objective To observe the changes in serum levels of CD5 antigen-like protein (CD5L) and neuronal positive pentamerin 2 (NPTX-2) in patients with acute cerebral infarction (ACI), and to explore the relationship between their expression and the severity ofcondition and their predictive value for short-term prognosis.Methods A total of 108 ACI patients admitted to the First Central Hospi-tal of Baoding from January 2021 to April 2023 were collected as observation subjects. According to the national institutes of healthstroke scale (NIHSS) score, they were separated into mild group (46 cases), moderate group (50 cases), and severe group (12 cases); ac-cording to the prognosis modified Rankin scale(mRS), they were separated into a poor prognosis group (26 cases) and a good prognosisgroup (82 cases). Clinical data of patients were collected; the levels of CD5L and NPTX-2 in the serum were detected using enzyme-linked immunosorbent assay (ELISA); correlation of CD5L, NPTX-2 levels with NIHSS scores and mRS scores in ACI patients was ana-lyzed by Spearman correlation; logistic regression was applied to analyze the influencing factors of short-term adverse prognosis in ACIpatients; receiver operating characteristic curve (ROC curve) was applied to analyze the diagnostic value of serum CD5L and NPTX-2 levels for short-term adverse prognosis in ACI patients.Results The levels of CD5L (147.11±19.85) ng/L and NPTX-2 (145.17±18.06) ng/L in the severe group were lower than those in the mild and moderate groups (P<0.05). The levels of CD5L and NPTX-2 in the mod-erate group (201.61±23.59) ng/L and (214.81±28.67) ng/L were lower than those in the mild group (215.83±28.64) ng/L and (232.97±30.08) ng/L (P<0.05). Compared with the group with good prognosis, the serum levels of CD5L and NPTX-2 in ACI patients with poor prognosis were obviously reduced (P<0.001). Spearman correlation analysis showed that CD5L levels were negatively correlated withboth NIHSS scores and mRS scores (r=.0.51,.0.54, P<0.05) and NPTX-2 levels were negatively correlated with both NIHSS scores and mRS scores (r=.0.47,.0.49, P<0.05). Serum CD5L and NPTX-2 were factors that affected poor prognosis in ACI patients (P<0.05). The area under the ROC curve (AUC) for short-term adverse prognosis in ACI patients diagnosed with serum CD5L, NPTX-2, and their combination was [0.86, 95%CI: (0.78, 0.92)], [0.87, 95%CI: (0.80, 0.93)], and [0.96, 95%CI: (0.90, 0.99)], respectively, the combined diagnostic efficacy of the two was higher than the individual detection efficacy (Z=2.41, P=0.016; Z=2.18, P=0.029).Conclusion The low expression of CD5L and NPTX-2 in serum of ACI patients may be closely related to their severity and prognosis, and the two andtheir combination have certain predictive value in short-term adverse prognosis of patients. |
