| 目的探讨高密度脂蛋白胆固醇降低对原发性高血压病人血压变异性的影响,方法该研究为临床回顾性研究,选取原发性高血压病人 406例,均来自宣城市人民医院 2019年 8月至 2021年 12月的住院病人,所有病人均行动态血压检查,根据高密度脂蛋白水平,分为高密度脂蛋白胆固醇降低组( <1.0 mmol/L)144例、高密度脂蛋白胆固醇正常组( ≥1.0 mmol/L)262例,该研究中血压变异性的指标: 24 h收缩压标准差和 24 h舒张压标准差,应用多因素线性回归分析高密度脂蛋白胆固醇降低对原发性高血压病人血压变异性的影响。结果与高密度脂蛋白胆固醇正常组比较,高密度脂蛋白胆固醇降低组 24 h收缩压标准差( 14.15±3.21)mmHg(1mmHg=0.133 kPa)、 24 h舒张压标准差( 11.27±2.41)mmHg,收缩压变异系数 0.105±0.020、舒张压变异系数 0.145±0.030均高于高密度脂蛋白胆固醇正常组的 24 h收缩压标准差( 12.96±2.51)mmHg、24 h舒张压标准差( 10.36±2.14)mmHg、收缩压变异系数 0.098±0.020、舒张压变异系数 0.133±0.030,差异有统计学意义(均 P<0.01)。先单因素线性回归分析后多因素线性回归分析显示:高密度脂蛋白胆固醇降低分组是原发性高血压病人 24 h收缩压标准差独立影响因素( P<0.001)。年龄、吸烟、高密度脂蛋白胆固醇降低分组是 24 h舒张压标准差独立影响因素( P<0.05)。结论高密度脂蛋白胆固醇降低是原发性高血压病人 24 h收缩压变异性及 24 h舒张压变异性的影响因素。 |

| Objective To investigate the effect of low high density lipoprotein cholesterol (HDL-C) on blood pressure variability in pa-tients with essential hypertension.Methods This study was a clinical retrospective study. According to the inclusion criteria, 406 pa-tients with essential hypertension, who were admitted to Department of Cardiology, Xuancheng People's Hospital from August 2019 toDecember 2021, were selected for the study. All patients completed ambulatory blood pressure examination. According to HDL-C levels,144 cases were assigned to low HDL-C group ( < 1.0 mmol/L) and 262 cases assigned to normal HDL-C group ( ≥ 1.0 mmol/L). Indicators of blood pressure variability in this study included 24-hour systolic blood pressure standard deviation and 24-hour diastolic blood pres-sure standard deviation. Multivariate linear regression analysis was used to analyze the effect of low HDL-C on blood pressure variability in patients with essential hypertension.Results Compared with the normal HDL-C group, the 24-hour systolic blood pressure standard deviation [(14.15±3.21) mmHg vs. (12.96 ± 2.51 ) mmHg](1 mmHg=0.133 kPa), 24-hour diastolic blood pressure standard deviation [(11.27±2.41) mmHg vs. (10.36 ± 2.14 ) mmHg], coefficient of variation for systolic blood pressure [(0.105±0.020) vs. (0.098 ± 0.020)], and coefficient of variation for diastolic blood pressure [(0.145±0.030) vs. (0.133 ± 0.030)] in low HDL-C group were higher; the differ-ences were statistically significant ( all P<0.001). Univariate and multivariate linear regression analyses results showed that low HDL-C group was an independent influencing factor for 24-hour systolic blood pressure standard deviation (P<0.001). Age, smoking, and low HDL-C group were independent influencing factors for 24-hour diastolic blood pressure standard deviation (P<0.05).Conclusion Low high density lipoprotein cholesterol is the influencing factor for 24 h systolic blood pressure variability and 24 h diastolic blood pressurevariability. |
