| 目的探究血清基质细胞衍生因子 -1(SDF-1)、人类 β-防御素 3(HBD3)、血管内皮生长因子受体 -1(sFlt-1)对重症肺炎(SP)合并呼吸衰竭病儿病情、预后的评估价值。方法选取 2021年 3月至 2022年 5月于上海交通大学医学院附属第九人民医院进行治疗的 104例 SP合并呼吸衰竭病儿为观察组,选择同时期来该院进行体检的 100例健康儿童为健康组。采用酶联免疫吸附分析试剂盒检测血清 SDF-1、HBD3、sFlt-1水平;分析三者血清水平与病儿病情程度的相关性;分析影响病儿预后的因素;受试者操作特征曲线( ROC曲线)分析三者血清水平对病儿预后的预测效能。结果观察组血清 SDF-1[(183.46±38.79)ng/ L比( 116.32±22.35)ng/L]、 HBD3[( 69.52±9.35)ng/L比(32.34±3.24)ng/L]、 sFlt-1[( 162.46±14.26)ng/L比( 96.21±6.85)ng/L]水平相比于健康组显著升高(t=15.07、37.65、42.03,P<0.05);血清 SDF-1、HBD3、sFlt-1水平与 SP合并呼吸衰竭病儿病情严重程度均呈正相关( r=0.46、0.48、0.43,P<0.05);预后不良组血清 SDF-1、HBD3、sFlt-1水平高于预后良好组( t=7.35、9.18、7.24,P<0.05),且血清 SDF-1、HBD3、sFlt-1均是影响 SP合并呼吸衰竭病儿预后的因素( P<0.05);血清 SDF-1、HBD3、sFlt-1及三者联合预测 SP合并呼吸衰竭病儿预后的曲线下面积( AUC)为 0.89、0.90、0.83、0.98,三者联合预测优于单独预测(均 P<0.05)。结论 SP合并呼吸衰竭病儿血清 SDF-1、HBD3、sFlt-1水平升高,并与病儿的病情严重程度和预后有关。 |

| Objective To explore the value of serum stromal cell-derived factor-1 (SDF-1), human beta-defensin 3 (HBD3), and vas-cular endothelial growth factor receptor-1 (sFlt-1) in assessing the condition and prognosis of children with severe pneumonia (SP) com-bined with respiratory failure.Methods A hundred and four children with SP combined with respiratory failure, who were treated inNinth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from March 2021 to May 2022, were selected asthe observation group, and 100 healthy children who came to the same hospital for physical examination in the same period were select-ed as the control group. An enzyme-linked immunoassay kit was used to detect the serum levels of SDF-1, HBD3 and sFlt-1; the corre-lation between the three serum levels and the severity of the children's disease was analyzed. The factors affecting the children's prog-nosis were analyzed, and the predictive efficacy of the three serum levels on the children's prognosis was analyzed by receiver operatingcharacteristic curve (ROC curve).Results The levels of serum SDF-1 [(183.46±38.79) ng/L vs. (116.32±22.35) ng/L], HBD3 [(69.52± 9.35) ng/L vs. (32.34±3.24) ng/L], and sFlt-1 [(162.46±14.26) ng/L vs. (96.21±6.85) ng/L] in the observation group were significantly higher than those in the control group (t=15.07, 37.65, 42.03, respectively; P<0.05). The levels of serum SDF-1, HBD3, and sFlt-1 were positively correlated with the severity of disease in children with SP complicated by respiratory failure (r=0.46, 0.48, 0.43, respectively; P<0.05). The levels of serum SDF-1, HBD3 and sFlt-1 in the poor prognosis group were higher than those in the good prognosis group (t =7.35, 9.18, 7.24, respectively; P<0.05). Furthermore, serum SDF-1, HBD3, and sFlt-1 were all factors affecting the prognosis of chil-dren with SP complicated with respiratory failure (P<0.05). The areas under the curve (AUCs) of serum SDF-1, HBD3, and sFlt-1, andall three in combination, for predicting the prognosis of children were 0.89, 0.90, 0.83, and 0.98, respectively, and the combined predic-tion of the three was superior to the prediction alone ( all P<0.05).Conclusion Serum SDF-1, HBD3, and sFlt-1 levels are elevated in children with SP combined with respiratory failure and are associated with the severity of the disease and prognosis of the children. |
