| 目的分析奥沙利铂致过敏性休克继发脑损伤的特点，为临床合理用药提供参考。方法 2024年 2月 1日至 4月 1日，上海交通大学医学院附属松江医院有 1例横结肠恶性肿瘤病人使用奥沙利铂导致过敏性休克继发脑损伤的案例，结合该病例查阅相关文献分析其原因及防治措施。结果根据病人的临床表现、药物不良反应及用药时间的关联性，考虑奥沙利铂导致的过敏性休克，经有效抗过敏治疗后，过敏性休克症状得到缓解，但造成了不可逆的脑损伤。结论临床应用奥沙利铂应充分了解病人既往奥沙利铂的用药史，识别高危人群，在病人化疗过程中需注意严格的医疗监护，警惕过敏性休克等严重不良反应，关注用药安全。 |

| Objective To analyze the characteristics of brain injury secondary to anaphylactic shock caused by oxaliplatin and to pro-vide reference for clinical rational drug use.Methods From February 1 to April 1, 2024, in Songjiang Hospital Affiliated to Shanghai Ji-ao Tong University School of Medicine, a case of brain injury secondary to anaphylactic shock caused by oxaliplatin was reported in a pa-tient treated for transverse colon malignant tumor. Based on the case, we reviewed the relevant literature to analyze the causes and preven-tive measures.Results According to the patient's clinical manifestations, adverse drug reactions (ADR) and the correlation with medica-tion time, anaphylactic shock was considered to be caused by oxaliplatin. After effective anti-allergic treatment, the symptoms of anaphy-lactic shock were relieved, but irreversible brain damage had occurred.Conclusion Prior to the clinical application of oxaliplatin, we need to review patients' history of oxaliplatin use, identify high-risk groups, carry out strict medical monitoring of patients during chemo-therapy, guard against serious adverse reactions such as anaphylactic shock, and attach great importance to the safety of drug use. |
