      目的探究调节性 B细胞( Breg)比例对妊娠期糖尿病( GDM)并发胎儿宫内生长受限( FGR)的影响及其机制。方法纳入 2020年 1月至 2024年 12月上海健康医学院附属崇明医院收治的 GDM病人 386例,按照超声诊断标准分为 FGR组( 76例)与非 FGR组( 310例)。通过流式细胞术、酶联免疫吸附分析( ELISA)、免疫组织化学、免疫共沉淀等实验验证 Breg及其分泌的白细胞介素 10(IL-10)与 GDM病人并发 FGR的关系。结果 FGR组外周血 Breg细胞比例( 13.11%±0.82%比 20.40% ±1.21%)及 IL-10含量[(40.02±0.45)mmol/L比( 65.47±0.60)mmol/L]显著低于非 FGR组,且 Breg比例、 IL-10浓度与母体糖化血红蛋白(HbA1c)呈负相关。胎盘免疫组化显示 FGR组 IL-10(+)细胞浸润及血管内皮细胞生长因子( VEGF)表达减少。 Breg来源的 IL-10可抑制胎盘螺旋动脉上皮细胞 VEGFR表达,且二者可相互结合。结论 GDM病人 Breg比例降低及功能紊乱,可能通过代谢 -免疫交叉调控影响胎盘血管生成,致使胎盘功能减退,是 GDM发生 FGR进展的潜在因素,未来需进一步探究相关治疗策略。
英文摘要:
      Objective To investigate the influence of regulatory B cell (Breg) proportions on GDM complicated with fetal intrauterinegrowth restriction (FGR) and explore its underlying mechanisms.Methods A analysis was conducted on 386 GDM patients in Shang-hai Health Medical College Affiliated Chongming Hospital from January2020 to December 2024, categorized into FGR (n=76) and non-FGR (n=310) groups based on diagnostic criteria. Flow cytometry, enzyme-linked immunosorbent assay (ELISA), immunohistochemis-try, and co-immunoprecipitation were employed to assess the association between Breg cells, interleukin-10 (IL-10), and FGR progres-sion in GDM patients.Results The FGR group exhibited significantly lower peripheral blood Breg cell proportions and IL-10 levels compared to the non-FGR group. Both Breg cell ratios and IL-10 concentrations were inversely correlated with maternal HbA1c levels. Placental immunohistochemistry revealed reduced IL-10(+) cell infiltration and decreased vascular endothelial growth factor (VEGF) expression in the FGR group. Mechanistically, Breg-derived IL-10 suppressed VEGFR expression in placental spiral artery endothelial cells, with direct binding observed between IL-10 and VEGFR.Conclusion The decrease and functional disorder of Breg ratio in pa-tients with GDM may affect placental angiogenesis and lead to placental dysfunction by metabolic-immune cstalk, which is a potentialfactor for the progression of FGR in GDM and further therapeutic strategies need to be further explored.
