      目的探讨创伤性骨折病人血清趋化因子配体 3(CCL3)、高迁移率族蛋白 Bl(HMGB1)水平与深静脉血栓( DVT)形成的相关性。方法纳入 2020年 5月至 2023年 6月于中国人民解放军联勤保障部队第九一〇医院就诊的 149例创伤性骨折病人作为研究对象,根据有无 DVT发生分为 DVT组( 31例)和无 DVT组( 118例)。采用 Pearson法分析血清 CCL3和 HMGB1表达水平的相关性;采用多因素 logistic回归分析与创伤性骨折病人并发 DVT有关的重要影响因素;采用受试者操作特征曲线(ROC曲线)评估血清 CCL3和 HMGB1表达对创伤性骨折病人发生 DVT的预测效能。结果 DVT组病人血清 CCL3[( 34.66±5.27)ng/L比( 29.83±4.32)ng/L]和 HMGB1[( 10.91±2.89)μg/L比( 7.75±1.75)μg/L]表达水平均高于无 DVT组( P<0.05);血清中 CCL3和 HMGB1二者的表达水平呈正相关( r=0.68,P<0.05); DVT组和无 DVT组年龄及骨折部位差异有统计学意义( P<0.05);年龄 ≥60岁( OR=2.33)、骨折部位为下肢( OR=2.00)、 CCL3(OR=2.44)和 HMGB1(OR=4.03)为创伤性骨折病人发生 DVT的危险因素( P<0.05);血清 CCL3、HMGB1以及二者联合检测的曲线下面积( AUC)分别为 0.71、0.84、0.93,联合高于各自单独检测(Z二者联合 -CCL3=3.54、Z二者联合 -HMGB1=2.73,P=0.004、0.006)。结论创伤性骨折病人血清 CCL3、HMGB1水平与 DVT的形成有关,二者联合对创伤性骨折病人发生 DVT具有一定的预测效能。

      Objective To investigate the correlation between serum levels of chemokine ligand 3 (CCL3) and high mobility group pro-tein Bl (HMGB1) in patients with traumatic fractures and the formation of deep vein thrombosis (DVT).Methods A total of 149 pa-tients with traumatic fractures admitted to 910th Hospital of Joint Logistic Support Force of PLA from May 2020 to June 2023 were col-lected as the research subjects, they were grouped into DVT group (31 cases) and non-DVT group (118 cases) based on the presence orabsence of DVT. Pearson method was applied to analyze the correlation between the expression levels of serum CCL3 and HMGB1;Multivariate logistic regression analysis was employed to identify significant influencing factors associated with DVT in patients withtraumatic fractures; receiver operating characteristic curve(ROC curve) was applied to analyze the predictive efficacy of serum CCL3and HMGB1 expression on the occurrence of DVT in patients with traumatic fractures.Results The serum expression levels of CCL3 [(34.66±5.27) ng/L vs. (29.83±4.32) ng/L] and HMGB1 [(10.91±2.89) μg/L vs. (7.75±1.75) μg/L] were higher in the DVT group than those in the non-DVT group (P<0.05); the expression levels of CCL3 and HMGB1 in serum were positively correlated (r=0.68, P<0.05); There were significant differences in age and fracture site between the DVT group and the non-DVT group (P<0.05). Age ≥60 years (OR =2.33), lower limb fracture site (OR=2.00), CCL3 (OR=2.44), and HMGB1 (OR=4.03) were identified as risk factors for DVT in patients with traumatic fractures (P<0.05). The area under the curve (AUC) for serum CCL3, HMGB1, and their combined detection were 0.71,0.84, and 0.93, respectively (Zcombination-CCL3=3.54, Zcombination-HMGB1=2.73, P=0.004, 0.006). Conclusion The levels of serum CCL3 and HMGB1 in patients with traumatic fractures are related to the formation of DVT, and the combination of the two has a certain predictiveeffect on the occurrence of DVT in patients with traumatic fractures.
