| 目的分析利培酮在儿童青少年精神障碍治疗中血液药物浓度监测结果及影响因素，为利培酮在儿童青少年病人中合理用药提供依据。方法回顾性收集 2018―2022年在深圳市康宁医院住院接受利培酮治疗并行血药浓度监测的儿童青少年精神障碍病人 243例，分析性别、年龄、给药剂量、是否原研产品、不同联合用药和联合用药品种数等对总活性成分血药浓度(CAM)、浓度剂量比( C/D)值和利培酮血药浓度 /9羟基利培酮血药浓度( CRIS/C9-OH-RIS)的影响。结果儿童青少年服用利培酮的日剂量为( 3.55±1.41)mg/d，CAM为 27.80 μg/L，C/D为 8.48。男性 CAM为 25.40 μg/L，C/D为 7.85；女性 CAM为 31.70 μg/L，C/D为 9.73。性别、日剂量和联合 3种精神药物对 C/D值有显著影响( P<0.01)。多重线性回归方程中，性别和日剂量对 CAM有显著影响( P<0.01)。结论利培酮治疗儿童青少年精神疾病时，受性别和日剂量的影响。临床医师或药师可应用血药浓度监测，结合临床疗效，指导调整用药。 |

| Objective To analyze the monitoring results and influencing factors of risperidone in children and adolescents with men-tal disorders, and to provide evidence for rational drug use of risperidone in children and adolescents.Methods The diagnosis andtreatment of 243 children and adolescents who received risperidone during their hospitalization in Shenzhen Kangning Hospital from2018 to 2022 were retrospectively collected. Effects of gender, age, dosage, presence of the originator product, different combinationdrug and number of varieties of co-medication on total active moiety concentration (CAM), dosage-related concentration (C/D) and risperi-done concentration/9-hydroxyrisperidone concentration (CRIS/C9-OH-RIS) were analyzed.Results The daily dose of risperidone in children and adolescents was (3.55±1.41)mg/d, the CAM value was 27.80 μg/L, and the C/D value was 8.48. The CAM value of males was 25.40 μg/ L,and the C/D value was 7.85. For females, the CAM values was 31.70 μg/L and the C/D value was 9.73. Gender, daily dose and combi-nation of 3 psychotropic drugs had significant effects on C/D value (P<0.01). Multiple linear regression showed that gender and daily dose had significant effects on CAM (P<0.01).Conclusions Risperidone for the treatment of psychiatric disorders in children and ado-lescents is influenced by gender and daily dose. Clinicians or pharmacists can use serum concentration monitoring, combined with clini-cal efficacy, to guide the adjustment of medication. |
