| 目的探讨自我效能感在类风湿关节炎病人疲乏与焦虑间的中介作用。方法选取 2023年 10月至 2024年 5月 5家医院[西北妇女儿童医院、空军军医大学第一附属医院(西京医院)、空军军医大学第二附属医院(第四军医大学唐都医院)、西安交通大学第一附属医院、西安交通大学第二附属医院]诊治的 265例类风湿关节炎病人为研究对象,采用一般资料量表、自我效能感量表、医院焦虑抑郁量表、 Bristol类风湿关节炎病人疲劳多维度问卷进行调查,运用描述性相关性分析及中介效应检验等方法进行数据分析。结果共发放问卷 265份,回收问卷 265份,回收率为 100%。医院焦虑抑郁量表的总分为( 7.51±1.67)分,焦虑得分( 4.03±1.53)分,抑郁得分( 3.48±1.36)分;疲乏量表的总分为( 14.88±2.13)分,躯体疲劳得分( 6.60±2.05)分,生活疲劳得分( 3.11±0.93)分,认知疲劳得分( 2.83±0.84)分,情感疲劳得分( 2.34±0.79)分;自我效能的总分为( 30.52±3.18)分。自我效能得分和焦虑、疲乏各分项指标得分均存在负相关关系( P<0.05)而焦虑、疲乏各分项指标得分间也彼此存在正相关关系( P<0.05)。自我效能在疲乏和焦虑抑郁之间具有中介作用,中介为 18.62%(P<0.05),直接效应占总效应的 81.38%。结论疲乏可以直接影响类风湿关节炎病人焦虑水平,也可以通过自我效能间接影响焦虑水平,因此,医护人员可借助体系化的干预效应,手段提升类风湿关节炎病人的自我效能,进而增强其治疗信心,缓解负面情绪。 |

| Objective To explore the mediating role of self-efficacy between fatigue and anxiety in patients with rheumatoid arthritis (RA).Methods A total of 265 RA patients diagnosed and treated in 5 hospitals (Northwest Women and Children's Hospital, XijingHospital, Tangdu Hospital, The First Affiliated Hospital of Xi'an Jiaotong University, and The Second Affiliated Hospital of Xi'an Jiao.tong University) from October 2023 to May 2024 were enrolled as the research subjects. Data were collected using the general informa.tion questionnaire, general self-efficacy scale (GSES), hospital anxiety and depression scale (HADS), and Bristol rheumatoid arthritis fa. tigue multi-dimensional questionnaire (BRAF-MDQ). Data analysis was performed using descriptive correlational method and media. tion effect test.Results A total of 265 questionnaires were distributed and all were recovered, with a 100% recovery rate. The totalscore of the HADS was (7.51±1.67), with a score of (4.03±1.53) for anxiety and (3.48±1.36) for depression. The total score of the fatiguescale was (14.88±2.13), including (6.60±2.05) for physical fatigue, (3.11±0.93) for daily life fatigue, (2.83±0.84) for cognitive fatigueand (2.34±0.79) for emotional fatigue. The total score of self-efficacy was (30.52±3.18). Self-efficacy scores were significantly negative. ly correlated with anxiety scores and all subscale scores of fatigue (P<0.05), while anxiety scores and all subscale scores of fatigue were significantly positively correlated with each other (P<0.05). Self-efficacy played a mediating role between fatigue and anxiety-depres. sion, with a mediating effect of 18.62% (P<0.05), and the direct effect accounted for 81.38% of the total effect.Conclusions Fatiguecan directly affect the anxiety level of patients with rheumatoid arthritis, and can also indirectly affect the anxiety level through self-effi. cacy. Therefore, medical staff can use systematic intervention methods to improve the self-efficacy of patients with rheumatoid arthritis, so as to enhance their confidence in treatment and relieve negative emotions. |
