| 目的探讨异时性滤泡性淋巴瘤( FL)和黏膜相关淋巴组织( MALT)结外边缘区淋巴瘤病人的临床表现、诊断、治疗过程及发病机制。方法总结胜利油田中心医院 2022年 8月收治的 1例异时性 FL和 MALT结外边缘区淋巴瘤病人的病历资料。结果病人于 2014年确诊 FL后经 R-CDOP(利妥昔单抗、环磷酰胺、脂质体阿霉素、长春新碱、泼尼松)方案化疗 10个周期，增强 CT疗效评价为完全缓解。 2021年 11月自觉视物模糊，后于 2023年 8月经病理确诊为: MALT结外边缘区淋巴瘤，予以 ZR2(利妥昔单抗、泽布替尼、来那度胺)方案化疗 5个周期、 OR2(利妥昔单抗、奥布替尼、来那度胺)方案化疗 1个周期，正电子发射计算机体层显像( PET/CT)疗效评价为完全代谢缓解，目前奥布替尼维持治疗中。结论异时性 FL和 MALT结外边缘区淋巴瘤临床表现为两种疾病各自症状序贯出现，依靠精确的病理结果诊断，治疗上需要高度个体化。因其在临床上较为罕见，有部分研究从 B细胞淋巴瘤 -2(Bcl-2)的恶性增殖潜能角度对其发病机制展开研究，值得临床进一步探讨。 |

| Objective To investigate the clinical manifestations, diagnosis, treatment process and pathogenesis of patients with meta.chronous follicular lymphoma (FL) and extranodal marginal zone cell lymphoma of mucosa-associated lymphoid tissue (MALT).Meth. ods The medical records were summarized of a patient with metachronous FL and extranodal marginal zone cell lymphoma of MALTadmitted to Shengli Oilfield Central Hospital in August 2022.Results The patient was diagnosed with FL in 2014, and then he under. went 10 cycles of R-CDOP (rituximab, cyclophosphamide, liposomal doxorubicin, vincristine, prednisone) chemotherapy. Enhanced CTevaluation indicated complete remission. In November 2021, the patient experienced blurred vision, and by August 2023, he was diag.nosed with extranodal marginal zone cell lymphoma of MALT. He received 5 cycles of ZR2 chemotherapy (rituximab, zanubrutinib, le.nalidomide) and 1 cycle of OR2 chemotherapy (rituximab, orelabrutinib, lenalidomide). Positron emission tomography and computed to.mography (PET/CT) assessment confirmed complete metabolic remission, and he is currently undergoing maintenance therapy with ore.labrutinib.Conclusions The clinical manifestation of metachronous FL and extranodal marginal zone cell lymphoma of MALT is thesequential occurrence of symptoms characteristic of each disease. Diagnosis is based on precise pathological results, and treatment re.quires a high degree of individualization. Because it is relatively rare in clinical practice, some studies have investigated its pathogenesisfrom the perspective of the malignant proliferation potential of B-cell lymphoma-2 (Bcl-2), which is worthy of further clinical exploration. |
