      目的探究血清血小板反应蛋白 -1(TSP-1)、可溶性 Fms样酪氨酸激酶 -1(sFlt-1)对全髋关节置换术( THA)后深静脉血栓(DVT)发生的临床预测价值。方法前瞻性选取邯郸市第一医院 2022年 2月至 2024年 3月行 THA病人 158例,将伴有 DVT的 55例病人纳入 DVT组,无 DVT的 103例病人纳入无 DVT组。采用酶联免疫吸附分析( ELISA)检测血清 TSP-1、sFlt-1表达水平;利用多因素 logistic回归分析 THA后 DVT发生的影响因素;利用受试者操作特征曲线( ROC曲线)研究血清 TSP-1、sFlt-1对 THA后 DVT发生的预测价值。结果与术前相比,术后血清 TSP-1、sFlt-1表达水平显著下调( P<0.05);与无 DVT组相比, DVT组 C反应蛋白( CRP)、血栓调节蛋白、凝血酶原时间均较高( P<0.05), TSP-1[( 1.46±0.47)μg/L比( 0.96±0.25)μg/L]、 sFlt-1[( 2.97±0.49)μg/L比( 2.51±0.32)μg/L]表达水平同样较高( P<0.05); CRP、TSP-1、sFlt-1均是影响 THA后 DVT发生的独立危险因素( P<0.05);血清 TSP-1、sFlt-1单独预测 THA后 DVT发生的 AUC分别为 0.78、0.83;二者联合预测的 AUC为 0.91,优于二者单独预测(均 P<0.05)。结论 TSP-1、sFlt-1在 THA后 DVT发生病人血清中表达上调,二者异常表达均影响着 THA后 DVT发生,二者联合对 THA后 DVT发生有着更高的临床预测价值。

      Objective To investigate the clinical predictive value of serum thrombospondin-1 (TSP-1) and soluble Fms-like tyrosine kinase 1 (sFlt-1) for deep vein thrombosis (DVT) after total hip arthroplasty (THA).Methods A total of 158 THA patients were pro. spectively selected from Handan First Hospital from February 2022 to March 2024. Fifty-five patients with DVT were included in theDVT group, while 103 patients without DVT were included in the non DVT group. Enzyme linked immunosorbent assay (ELISA) wasapplied to detect the expression levels of serum TSP-1 and sFlt-1. Multivariate logistic regression was applied to analyze the influenc.ing factors of DVT occurrence after THA. Receiver operating characteristic curves (ROC curves) were applied to investigate the predic.tive value of serum TSP-1 and sFlt-1 for the occurrence of DVT after THA.Results Compared with the preoperative period, the serum expression levels of TSP-1 and sFlt-1 were significantly down-regulated (P<0.05). Compared with the non DVT group, the DVT group had greatly higher C-reactive protein (CRP), thrombomodulin, prothrombin time (P<0.05), and the expression levels of TSP-1 [(1.46± 0.47) μg/L vs. (0.96±0.25) μg/L] and sFlt-1 [(2.97±0.49) μg/L vs. (2.51±0.32) μg/L] were also significantly elevated (P<0.05). CRP, TSP-1, and sFlt-1 were all independent risk factors for the occurrence of DVT after THA (P<0.05). The AUC values predicted by serum TSP-1 and sFlt-1 alone for the occurrence of DVT after THA were 0.78 and 0.83, respectively. The AUC predicted by the combinationof the two was 0.91, which was better than the prediction of the two alone (both P<0.05).Conclusions TSP-1 and sFlt-1 are upregulat.ed in the serum of patients with DVT after THA, and their abnormal expression affects the occurrence of DVT after THA. The combina.tion of the two has higher clinical predictive value for the occurrence of DVT after THA.
